摘 要:目的研究大鼠放射性肺损伤CT肺功能定量分析及其与肺功能试验的相关关系,初步探讨CT定量分析对于放射性肺损伤的早期诊断价值。材料与方法将60只SPF级健康雌性SD大鼠,随机等分为两组(对照组和实验组)。实验组采用60Co右肺单分割照射20 Gy,另一组为空白对照组。于照射后第2、4、6、8、12、16周行高分辨率CT(HRCT)扫描并作CT肺功能定量分析,每组随机抽取5只大鼠测定肺功能试验指标,行HE染色比较观察其病理学变化。结果大鼠照射后改变仅发生在照射野内且影像学表现及病理学改变与人相似。照射组大鼠第2~16周右肺平均肺密度均值高于对照组,且Pulmo曲线峰值右移。大鼠放射性肺损伤CT肺功能定量分析与肺功能试验指标部分相关,其中用力肺活量(FVC)与像素指数(PI)-800~-700相关性最好(r=0.936,P<0.01)。结论放射性肺损伤大鼠的CT肺功能定量分析指标与肺功能试验指标(PFT)有一定的相关性,对于早期放射性肺损伤的诊断有较高的价值。Objective To study the correlation of quantitative CT with pulmonary function tests(PFT) in radiation induced lung injury of rats,and to investigate the values of the quantitative CT analysis in the early diagnosis of radiation induced lung injury.Materials and Methods Sixty SPF health female SD rats were divided into 2 groups,one radiated groups and one normal control group randomly.Right lung in the radiated group rat received a dose of 20Gy with 60CO.The rats were underwent high resolution computed tomography(HRCT) scan,CT quantitative lung function and PFT index analysis at the 2nd,4th,6th,8th,12th,16th week after establishing lung injury model.The rats's lung was fixed and HE stained for pathology.Results After irradiation,the pathological changes only occurred in radiate field.The pathological and HRCT signs on rats were similar to that on human.At 2nd ~16th week after irradiation,the average density of right lungs in radiation groups was higher than that of normal control group(P﹤0.05),and Pixel Index(PI) curve peak shifted to the right.There was correlation between quantitative CT lung function and PFT in radiation induced lung injury rat model,especially FVC and PI-800——700,have highest correlation(r=0.936,P0.01).Conclusion There is correlation between quantitative CT lung function and PFT in radiation induced lung injury rat model,which can be benefit in early diagnosis of radiation induced lung injury.
