术中椎管造影用于急性胸腰段爆裂骨折20例分析

摘　　要：目的探讨术中椎管造影在后路内固定治疗急性胸腰段爆裂骨折的指导意义。方法选择20例急性胸腰段爆裂骨折患者,均行开放后路内固定,手术时先行内固定复位,然后经切口行椎管造影,通过C臂X线光机透视下了解骨折块复位椎管通畅情况。对骨折不能完全复位,椎管不通畅者行椎板切开、探查减压。结果 20例患者中有15例椎管通畅,骨折块得以回纳,未行椎板切开减压;5例显示不通畅而行椎板切开减压。所有患者均有半年以上的随访,神经功能均有所恢复。结论后路治疗急性胸腰段骨折,术中椎管造影具有指导意义且容易实施,简单、直观、费用低等优点,是一种值得推广的方法。Objective To explore the significance of intraoperative intradiscal imaging posterior-instrumentation operations in patients with acute thoracolumbar burst fractures.Methods 20 cases of acute thoracolumbar burst fractures were selected,posterior open fixation were performed for all patients.Firstly,internal fixation was reseted,then the incision line intradiscal imaging through the X-ray machine C arm fluoroscopy could understand the block fracture reset.If the fracture couldn＇t completely reset,exploring and reducing the pressure had to be done.Results 15 cases were intradiscal unobstructed and whose the block fracture was returned.They did not cut lamina for decompression.5 cases showed no unobstructed and cut lamina for decompression.More than half of all cases were followed-up,neural function all been restored.Conclusion For treated acute thoracolumbar burst fractures patients,intraoperative intradiscal imaging is a kind of popularization worth method with easy to implement,simple,intuitive,and less spend etc.
