联合超声技术对下肢深静脉血栓的研究

摘  要:目的探讨下肢深静脉血栓(DVT)的超声应用价值。方法对45例单侧下肢临床高度怀疑DVT的患者进行超声检查,包括二维加压超声、彩色血流多普勒显像(CFDI)、二维血流显像(B-flow)及脉冲多普勒技术(PWD)的综合使用。结果 34例经检查确诊DVT,其中4例合并有浅静脉血栓,9例确诊为血液高凝状态。完全栓塞42个部位,不完全栓塞21个部位。结论加压超声结合CDFI及B-flow对确定静脉血栓部位、静脉阻塞、再通情况、血液高凝状态的判断有可靠的价值,为诊断DVT的首选方法,PWD对判断血流状态有帮助,但对判断血栓的存在与否不是必须的。Objective To assess the application value of ultrasonographic methods for limbs deep venous thrombosis(DVT).Methods 45 consecutive patients with signs and symptoms that were clinically more suggestive of DVT were examined by US.Four ultrasonography techniques werein use,including compression ultrasonography,Color Doppler Flow Imaging(CDFI),B-flow imaging(B-flow) and pulse wave Doppler(PWD) facility.Results Diagnosis were confirmed in 34 subjects veins by US,among them,4 subjects were suffered from deep venous thrombosis(DVT) accompanied with superficial venous thrombosis.9 were blood hypercoagulability.42 veins were complete obstruction,21 incomplete.Conclusion Compression ultrasonography associated with CFDI and B-flow imaging should be the preferred initial diagnostic test for patients with clinically suspected DVT.PWD is helpful to identify conditions of blood flow but is not mandatory.
