复肝降糖方对肝硬化伴肝源性糖尿病患者胰岛素抵抗及瘦素影响的临床研究

摘　　要：目的观察复肝降糖方对肝硬化伴肝源性糖尿病患者胰岛素抵抗及瘦素的影响。方法 60例肝硬化伴肝源性糖尿病患者,随机分为治疗组和对照组各30例。两组均采用常规护肝降酶基础治疗,对照组加用阿卡波糖片口服,50mg/次,3次/d;治疗组加服复肝降糖方,每日1剂,分2次服;疗程均为3个月。观察治疗前后肝功能(ALT、AST、TBiL),空腹血糖(FPG),空腹胰岛素(FINS),糖化血红蛋白(HbA1c),胰岛素抵抗指数(HOMA-IR)及瘦素(Leptin)指标的变化。结果两组治疗后ALT、AST、TBiL均较治疗前明显下降(P<0.01),治疗组治疗后ALT、AST下降明显优于对照组(P<0.01);两组治疗后FPG、FINS、HOMA-IR及HbA1c均较治疗前明显改善,差异有统计学意义(P<0.01);治疗组治疗后FPG、HbA1c下降明显优于对照组(P<0.01),FINS、HOMA-IR改善亦明显好于对照组,差异有统计学意义(P<0.01);两组治疗后瘦素均较治疗前明显降低,差异有统计学意义(P<0.05或0.01),治疗组治疗后LPN比对照组下降更显著(P<0.05)。结论复肝降糖方治疗肝硬化伴肝源性糖尿病可改善肝功能,有效降低血糖、血清瘦素水平。Objective To observe the efficacy of Fufang Jiangtang formula influencing insulin resistance and leptin in patients with hepatic diabetes induced by cirrhosis. Methods Sixty Patients with Hepatic Diabetes induced by cirrhosis were randomly divided into treatment group （n = 30） and control group （n = 30）. During the trial period, all of patients received a basic treatment with routine protecting liver and lowering transaminase drugs. Meanwhile, control group were added acarbose tablets 50mg per time, three times per day, and the treatment group added Fufang Jiangtang formula, one dose per day, a half in the morning and the other in the afternoon respectively. The course of treatment was all 3 months, the levels of ALT, AST, TBiL, FPG, FINS, HbAlc, HOMA - IR and Leptin will be compared pre - and post therapy. Results After treatment, the levels of ALT, AST and TBiL in two groups were obviously descended （P 〈 0.01 ）, and compared the descended levels of ALT and AST, the treatment group was superior to control group （ P 〈 0. 01 ） ; Comparing the levels of FPG, FINS, HbAlc, HOMA-IR and Leptin in two groups pre - and post therapy suggested an obviously ameliorate （P 〈 0. 05 or 0. 01 ）. The treatment group was also superior to control group （ P 〈 0. 05 or 0. 01 ）. Conclusion Fufang Jiangtang formula treating Hepatic Diabetes induced by cirrhosis could improve the liver function, hyperinsulinism and insulin resistance, also efficiently lower the levels of blood glucose and Leptin.
