保护腰椎管内硬脊膜外脂肪在腰椎间盘手术中的应用

摘　　要：目的:探讨手术中保护椎管内硬膜外脂肪组织,尽量减少的影响解剖结构关系,减轻术后神经根粘连在治疗椎间盘突出髓核摘除患者中的应用。效果好,并发症及术后残留症减少。方法:无论是椎板开窗或是半椎板切除髓核摘除术,均采用不切除硬膜外脂的手术进行对比,对所有患者进行随访分析。结果:206例进行随访3～42个月,平均13个月,根据Macnnab标准,优良率97.1%。结论:保护硬膜外脂肪在椎间盘手术中的应用具有明显的低术后并发症的优点,疗效确切。Purpose：It is to make a study of the protection of lumbar spinal epidural fat tissue in the operation,minimize the effect on anatomical structural relation and lighten root adhesion after lumbar intervertebral disc operation in the treatment of the patient who has accepted a lumbar intervertebral disc protrusion ablation.The advantages are good efficiency and less complication and sequelae.Method：For vertebral-plate open discectomy or half vertebral plate open and nucleus pulposus excise,the reservation of spinal epidural fat is adopted for comparison,to give follow-up analysis fir all patients.Result：206 cases are followed up in 3-42 months,averagely 13 months.As per Macnnab standard,the excellent rate is 97.1%.Conclusion：the protection of lumbar spinal epidural fat has obvious advantage of less complication and definite effect in the intervertebral disc operation.
