TCT检查正常妇女HPV的消退规律及其与CIN的关系

摘　　要：目的:探讨HR-HPV及LR-HPV的消退规律,为细胞学检查正常的HPV阳性人群制定合理的个体化诊疗方案。方法:对2007年1月～2008年12月在吉林大学第二医院妇科门诊妇科体检宫颈细胞学检查正常、HPV检测阳性的妇女进行定期随访,对6、12、18、24个月做TCT、HPV检测的316例患者资料进行回顾性分析。结果:随访至第6、12、18、24个月HR-HPV消除率分别为47.51%、58.37%、64.71%、73.30%,LR-HPV消除率分别为86.32%、92.63%、94.74%、97.89%。结论:HR-HPV感染后2年基本消退,LR-HPV感染后半年基本消退。HR-HPV持续阳性妇女易发生CINⅡ-Ⅲ,故宜严密随访。Objective：To explore the regularities of high risk（HR）-human papillomavirus（HPV） clearance and low risk（LR）-HPV clearance,design a reasonable and individualized diagnostic and therapeutic scheme for HPV positive women with normal cytological results. Methods：The HPV positive women with normal cervical cytological results who received physical examination in gynecological outpatient of the hospital from January 2007 to December 2008 were followed up regularly,the data of 316 patients receiving thin layer liquid-based cytological test（TCT） and HPV detection at 6,12,18 and 24 months were analyzed retrospectively. Results：The clearance rates of HR-HPV at 6,12,18 and 24 months were 47.51%,58.37%,64.71% and 73.30%,respectively;the clearance rates of LR-HPV at 6,12,18 and 24 months were 86.32%,92.63%,94.74% and 97.89%,respectively. Conclusion：HR-HPV degrades basically at 2 years after infection,LR-HPV degrades basically at half a year after infection.The women with persisting positive HR-HPV are prone to CINⅡ～Ⅲ,so close follow-up is suggested.
