围术期舒适护理对剖宫产患者应激反应的影响

摘　　要：目的:比较围术期剖宫产患者实施舒适护理和常规护理后对血浆皮质醇和β-内啡肽的影响。方法:将行剖宫产患者120例随机分为舒适护理组和常规护理组,每组60例。常规组围术期给予基础护理及产科专科护理,舒适组在常规组基础上给予舒适护理。术毕患者行硬膜外自控镇痛,检测术前1小时、术后4、16、24、48小时血浆皮质醇和β-内啡肽浓度。结果:舒适组术前1小时、术后4小时和16小时的血浆皮质醇及β-内啡肽浓度明显低于常规组(P<0.05);而术后24小时以后两组血浆皮质醇和β-内啡肽浓度差异无统计学意义(P>0.05)。结论:围术期舒适护理能有效抑制剖宫产患者应激反应,是剖宫产手术较为理想的护理方法。Objective：Comparison perioperative cesarean patient with comfort care and routine care after plasma cortisol and β-endorphin.Methods：120 patients were divided into two groups,Conventional group-based perioperative care and specialist care,comfortable conventional group patients based on given comfort care.plasma concentrations of cortisol and β-endorphin at 1h before operation,4h,16h,24h,48h after operation were assayed.Results：Comfortable preoperative 1h,4h and 16h after β-endorphin cortisol concentrations were significantly lower than conventional group;24h after surgery,two groups plasma cortisol and β-endorphin concentration difference was not statistically significant.Conclusion：Perioperative comfort care can effectively relieve stress reaction and it may be a better nursing method in caesarean section patients.
