摘 要:目的探讨定量组织速度成像(QTVI)技术评价甲状腺功能亢进(甲亢)性心肌病兔不同左室构型心功能变化的价值。方法新西兰纯种白兔30只,随机分为实验组(20只)和对照组(10只)。实验组腹腔注射左旋甲状腺素[45μg/(kg·d)]×28d建立甲亢动物模型,实验组依据Ganau的超声参数分为两型:向心性肥厚组(CH组)和离心性肥厚组(EH组),各组均行常规超声心动图及QTVI参数测定,包括:二尖瓣环收缩期峰值速度(Vs)、收缩期加速度(a)和舒张期峰值速度(Ve)、E/Ve等。结果 CH组左室质量指数(LVMI)及相对室壁厚度(RWT)明显增加(P<0.01);LVEF、FS及E峰与对照组比较差别不具显著性意义(P>0.05)。EH组左室质量指数(LVMI)明显增加(P<0.01),而相对室壁厚度(RWT)与对照组比较差别不具显著性意义(P>0.05);LVEF、FS及E峰与对照组比较明显减低(P<0.01)。CH组与对照组比较,二尖瓣环平均Ve减低(P<0.01),E/Ve增加(P<0.01),但平均Vs、a与对照组比较差异不具显著性意义(P>0.05)。EH组与对照组比较,平均Vs、a明显减低(P<0.01);平均Ve明显减低(P<0.01),E/Ve明显增加(P<0.01)。结论 QTVI技术可具体发现甲亢性心肌病兔不同左室构型左室收缩及舒张功能的变化情况。Objective To evaluate the left ventricle function of different geometic patterns in rabbits with hyperthyroid cardiomyopathy by quantitative tissue velocity imaging (QTVI). Methods Thirty new Zealand purebred rabbits were randomly divided into two groups ,. control group (n= 10) and experimental group (n=20). Hyperthyroidism animal model was established by peritoneal injection of Levothyroxine (L Thy) in experimental group. According to the ultrasound parameter changes,the ventricular geometries of experimental group were divided into two groups:concentric hypertrophy (CH) and eccentric hypertrophy(EH). Peak velocity during systole (Vs), systolic acceleration (a), peak velocity during diastole (Ve) and E/Ve at the mitral annulus were measured by QTVI and conventional echocaidiographic parameters were also obtained. Results In CH group,the LVMI and RWT increased obviously(P〈0.01). In EH group,the LVMI increased obviously(P〈0.01), but RWT did not change sxgnificantly(P〈0.05). EF and FS were significantly lower in EH group than those of the control group(P〈0.01). Average Ve was significantly lower in CH group than that of the control group(P〈0.01). Average Vs,a and Ve were significantly lower in EH group than those of the control group(P〈0.01). E/Ve was significantly higher in CH and EH groups than that of the control group(P〈0.01). Conclusions QTVI can accurately assess the left ventricle function of different geometic patterns in rabbits with hyperthyroid cardiomyopathy.
