常规超声、超声造影及增强CT对肾恶性肿瘤诊断的比较

摘　　要：目的比较常规超声、超声造影及增强CT对肾恶性肿瘤的诊断价值。方法对42例肾恶性肿瘤患者术前行常规超声、超声造影和增强CT检查,并与术后病理结果对比。结果肾恶性肿瘤中超声造影无增强区的显示率明显高于常规超声无回声的显示率(P<0.05);超声造影假包膜的显示率明显高于常规超声(P<0.05);超声造影血流丰富或抱球型血流的显示率明显高于常规超声(P<0.05);超声造影及增强CT对肾恶性肿瘤诊断的准确率明显高于常规超声诊断的准确率(P<0.05),超声造影及增强CT诊断的准确率之间比较无统计学差异。结论与常规超声相比,超声造影增加了肾恶性肿瘤内无增强区、假包膜及抱球型血流的显示率;超声造影提高了肾恶性肿瘤的超声诊断准确率,与增强CT相近。Objective To compare the diagnostic value of conventional ultrasonography, contrast-enhanced ultrasonography（CEUS） and contrast-enhanced CT in the renal malignancy. Methods Forty two cases of malignant renal tumor underwent conventional ultrasonography, CEUS and contrast-enhanced CT before surgery were selected, and compared the results with the pathology results after surgery. Results The positive rate of no enhanced area in CEUS is obviously higher than that in conventional ultrasonography （P〈0.05）. The positive rate of the pseudocapsule in CEUS is obviously higher than that in conventional ultrasonography （P〈0.05）. The positive rate of rich blood flow or blood flow ring around the tumor in CEUS is significantly higher than that in conventional ultrasonography （P〈 0.05）. Compared with the pathology results, the accurate diagnosis rates in the CEUS and contrast-enhance CT are significantly higher than that in the conventional ultrasonography（P〈0.05）. There is no difference between CEUS and contrast-enhance CT. Conclusions Compared with conventional ultrasonography, CEUS can show higher positive rate of no enhanced area, pseudocapsule, rich blood flow or blood flow ring around the tumor. CEUS can increase the diagnostic accuracy in the the renal malignancy,that is similar to contrast-enhance CT.
