摘 要:目的:建立一种新型腰椎间盘突出致坐骨神经痛大鼠模型和硬膜外腔置管方法。方法:60只雄性SD大鼠随机分为模型组(n=36),假手术组(n=12)和空白组(n=12),分别行模型制作,假手术,不做任何处理。每组6只大鼠于术前1天及术后第1、3、7、14、21、28天做一般行为学观察及50%机械性撤足阈值(50%PWT)测定。每组6只大鼠于术后第7天检测术侧L5背根神经节(DRG)中NOS和COX-2的表达。模型组中另外24只大鼠于术后第2、6、13、20天各取6只行导管位置和硬膜外腔药液扩散程度的确定。结果:所有大鼠均未出现运动及大小便功能障碍。模型组术后各观测点50%PWT比术前,空白组和假手组明显降低(P<0.05)。模型组术后第7天术侧L5 DRG的NOS和COX-2阳性细胞数比空白组和假手术组明显升高(P<0.05)。所有导管均位于硬膜外腔内,术后第6天后扩散节段稳定(P>0.05)。结论:本研究建立的新型腰椎间盘突出致坐骨神经痛大鼠模型制作简便,机械痛敏稳定,炎症反应确切,硬膜外腔置管方法可行。Objective: To establish new rat model for sciatica induced by lumbar disc herniation and new method for epidural catheterization.Methods: 60 male SD rats were randomly divided into model group(n = 36),sham group(n = 12) and blank group(n = 12).Autologous nucleus pulposus was applied to the L5 dorsal root ganglion(DRG) and epidural space and PE-0503 catheter was put into epidural space in model group.Autologous nucleus pulposus was harvested but not applied to the L5 DRG or epidural space in sham group.There was nothing operation in blank group.General behaviors were observed and 50% paw withdrawal threshold(50%PWT) was measured 1 day before the surgery and on days 1,3,7,14,21,and 28 after surgery in each group with 6 rats.On day 7 after surgery,the left L5 DRGs were obtained for detecting the expression of NOS and COX-2 in each group with 6 rats.On days 2,6,13 and 20 after surgery in model group(6 rats each day),rats were dissected to evaluate the position of the catheter tips and the extention of drug in epidural space.Results: No motor,urination and defecation disfunction were observed.50%PWT after surgery were significantly lower than 1 day before surgery,blank group and sham group(P 0.05).Significant increase of positive cell numbers of NOS and COX-2 was observed in model group(P 0.05).The catheter tips were all at epidural space and the extention of durg in epidural space after day 6 was stable(P 0.05).Conclusion: This new rat model for sciatica induced by lumbar disc herniation was reliable with stable mechanical hyperalgesia and assured inflammatory response,and the epidural catheterization was proved to be feasible.
