自身冷抗体伴同种抗体的重症β地中海贫血患者一例的血型血清学分析

摘　　要：目的分析1例罕见的自身抗体伴同种抗体重症β地中海贫血患者的血型血清学特点。方法在室温和37℃条件下分别在盐水、凝聚胺和抗人球介质中进行不规则抗体筛选;利用吸收放散实验处理患者红细胞和血清,排除自身冷抗体干扰后进行不规则抗体鉴定、血型鉴定和交叉配血。结果在此例重症β地中海贫血患者体内鉴定出高效价的自身冷抗体,并伴同种抗E、抗N抗体。结论当发现重症β地中海贫血患者血型正反不合和交叉配血困难时要及时进行血型血清学鉴定,以免误输不相合的血液而加重病情。Objective To analyze the blood group serology characteristics of one case β-severe thalassemia patient with autoantibodies and alloantibodies.Methods At room temperature and 37 ℃,irregular antibody screening was performed in the saline,polybrene and anti-human globulin media,respectively.After excluding cold autoantibodies interference with the absorption-elution test,irregular antibody screening,blood group identification and cross matching were performed.Results High titer cold autoantibodies and alloantibodies（anti-E and anti-N） were both found in this thalassemia patient.Conclusion When difficulties in blood group identification and cross matching were found in severe thalassemia patients,blood group serology analysis will help to avoid the transfusion accident induced by the undetected antibodies.
