摘  要:目的通过检测新生儿缺氧缺血性脑病(HIE)治疗前、后血清神经元特异性烯醇化酶(NSE)水平,探讨其在HIE中的临床价值。方法采用电化学发光法测定90例HIE患儿治疗前、后及30名正常对照儿童血清NSE水平。将90例HIE患儿按诊断标准分为轻度、中度、重度HIE组,比较各组治疗前、后NSE水平的差异。结果 HIE患儿血清NSE水平明显高于对照组(P<0.01),且HIE临床分度越重,血清NSE水平升高越明显(P<0.01)。HIE各组患儿治疗后NSE水平明显低于治疗前(P<0.05、P<0.01)。结论测定血中NSE能及时、有效地帮助临床对新生儿HIE的脑损伤做出早期诊断和病情轻重的判断,为治疗的监测和预后提供可靠的指导参考。Objective To evaluate the clinical value in neuron-specific enolase(NSE) by the detection of the NSE levels in serum of neonates with hypoxic ischemic encephalopathy(HIE) before and after the treatment.Methods The 90 neonates with HIE and 30 healthy children were enrolled,and the levels of serum NSE before and after treatment were determined by chemiluminescence immunoassay.The 90 neonates with HIE were classified into light HIE,middle HIE and severe HIE groups according to the diagnosis standard,and the differences among the groups before and after the treatment were analyzed.Results Compared with the controls,the levels of serum NSE were significantly higher in neonates with HIE(P0.01).The more severe the clinical degree of HIE was,the more significantly the levels of serum NSE increased(P0.01).The levels of HIE after the treatment were significantly lower than those before the treatment(P0.05,P0.01).Conclusions The determination of serum NSE can help the early clinical diagnosis and judgement of the severity of neonatal HIE brain damage timely and effectively,and offer a reliable reference for treatment monitoring and prognosis.
