摘 要:目的探讨厄贝沙坦/氢氯噻嗪对原发性高血压伴早期肾损害患者尿微量白蛋白(mALB)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG酶)影响。方法将80例原发高血压(1~2级)伴早期肾损害患者按就诊顺序随机分为厄贝沙坦/氢氯噻嗪组40例和硝苯地平控释片组40例,均服药12周。观察两组用药前后血压、尿mALB、NAG酶、肾功能、血糖、血脂、血钾的变化。结果两组经12周治疗后血压均较治疗前明显降低(P<0.01),但两组治疗后血压比较,差异无统计学意义(P>0.05);两组治疗前后肾功能、血糖、血脂、血钾比较,差异无统计学意义(P>0.05);两组尿mALB、NAG酶均较治疗前降低(P<0.05),厄贝沙坦/氢氯噻嗪组明显优于硝苯地平控释片组(P<0.05)。结论厄贝沙坦/氢氯噻嗪具有良好的降压作用,同时能降低尿mALB、NAG酶的水平,改善肾功能。Objective To investigate the effects of irbesartan /hydrochlorothiazide for hypertension in patients with early renal damage in urinary albumin(mALB),N-acetyl-β-D-glucosaminidase(NAG enzyme).Methods Randomly divided 80 patients with primary hypertension(grade 1~2)who were with early renal injury also into two groups,one group of 40 patients treated with irbesartan/hydrochlorothiazide,the other of 40 patients with nifedipine,by the sequence of their treatment,medication for 12 weeks.Observed the data and changes in blood pressure,urine mALB,NAG enzymes,kidney function,blood glucose,blood lipids and serum potassium before and after the medication.Results The blood pressure of the two groups was significantly lower than that of before the treatment(P<0.01),but when compared the blood pressure of two groups of patients after treatment,the statistically difference was not significant(P>0.05);the renal function,blood glucose,blood lipids,serum potassium changes before and after the treatment were not statistically significant(P>0.05);the urinary mALB,NAG enzyme of two groups decreased than those of before the treatment(P<0.05),but the group of irbesartan /hydrochlorothiazide was significantly better than that group of nifedipine(P<0.05).Conclusion Irbesartan/hydrochlorothiazide has a good antihypertensive effect and can reduce the levels of urinary mALB,NAG enzyme at the same time,and improve the renal function.
