摘 要:目的总结西北地区15家教学医院2010年度临床分离病原菌的耐药状况。方法常规方法培养分离医院内感染病原菌,并应用半自动或全自动细菌鉴定分析仪鉴定到种,药敏试验方法按CLSI规定的标准进行。监测数据按卫生部全国细菌耐药监测(Mohnarin)中心设计方案的要求定期上报。用WHONET5.6软件进行数据统计分析。结果西北地区15家教学医院共分离出病原菌株23154株,其中革兰阴性菌16473株(占71.1%),革兰阳性菌6681株(占28.9%)。铜绿假单胞菌和鲍曼不动杆菌对多粘菌素的敏感率为96%左右。耐甲氧西林的金黄色葡萄球菌(MRSA)与凝固酶阴性葡萄球菌(MRSCoN)的发生率分别为55.1%和89.8%,未发现对万古霉素和利奈唑胺耐药的葡萄球菌。凝固酶阴性葡萄球菌对替考拉宁的耐药率为0.3%。粪肠球菌、屎肠球菌对氨苄西林的耐药率分别为25.8%,88.2%;粪肠球菌对替考拉宁和万古霉素耐药率分别为1.1%,1.0%;屎肠球菌对替考拉宁和万古霉素耐药率分别为1.5%,0.6%。结论细菌的耐药性有逐年上升的趋势,而且耐药现象存在地区差异。MRS、多药耐药的铜绿假单胞菌、泛耐药的鲍曼不动杆菌仍然是耐药监测值得关注的病原菌。Objective To summarize 2010 annual clinical isolated - drug resistance in Northwest of China with eleven teaching hospitals. Methods Isolates were cultured by routine method and identified by the semi - automatic or automatic bacteria analysis system. Drug susceptibili- ty test was according to CLSI standards. Supervising data delivered to MOH National Antimicrobial Resistant Investigation Net (Mohnarin) according to its plan. Data statistic and analysis used by WHONET 5.6 software. Results A total of 23154 strains pathogens were isolated in Northwest area with eleven teaching hospital, in which gram negative bacteria had 16473 strains (71.1%) , gram positive bacteria had 6681 strains (28.9%). The drug susceptible rate of Pseudomonas aeruginosa and Acinetobacter baumannii against polymyxin were 96%. Methicillin - resistant S. aureus (MRSA) were 55.1% , MRSE were 89.8%. Resist- ant vancomycin and linezolid to staphylococcus didn't detect. The drug susceptible rate of Staphylococcus epicermidis against ampieillin was 25.8% and Staphylococcu heamolyticus was 88.2%. The drug resistant rates of Enterococcus faecalis against teicoplanin and vancomycin were 1.1% and 1.0%. Conclusion The bacterial resistance is still on the rise. And the resistance has regional differences. Much attention should be paid to MRS, multi -drug resistance Pseudomonas aeruginosa and pan -drug resistance Acinetobacter baumannii.
