摘  要:目的探讨一种新的治疗方法处理中颅窝底巨大海绵窦海绵状血管瘤,以提高手术治疗效果。方法回顾性分析2例中颅窝底巨大海绵窦海绵状血管瘤的临床资料,术前均行放疗,放疗后1—2个月行手术治疗。结果放疗后病灶体积明显缩小,术中输血量显著减少,术后随访动眼神经和视神经功能恢复良好,MRI检查证实病灶完全切除。结论对中颅窝底巨大海绵窦海绵状血管瘤行先放疗后手术的治疗策略,对减少术中出血、避免脑神经损伤、提高手术切除率具有重要意义。Objective To explore a new effective technique in microsurgical treatment for giant eaverous hemangioma in cavernous sinus. Method The clinical data of 2 cases of giant caverous hemangiomase (GCH)in cavernous sinus were retrospectively analyzed. Preoperation radiotherapy were performed in both patients and after 2 to 4 weeks the operation was performed. Results The volume of GCHs was obviously decreased and intraoperative blood transfusion was significantly reduced after radiotherapy. Both GCHs were completely removed comfirmed by MRI examination. The functions of oculomotor nerve and optic nerve recovered well after operation. Conclusions Operation after radiotherapy is effective for treatment of giant caverous hemangiomas in cavernous sinus . Decreasement of intraoperative bleeding, prevention of cerebral nerves injury and increasement of the radio of total resection could be achieved.
