摘　　要：目的探讨孕激素地屈孕酮对先兆流产患者Th1/Th2型细胞因子平衡的影响,分析以免疫调节治疗先兆流产的机制。方法采用酶联免疫吸附方法检测42例正常妊娠妇女和60例先兆流产妇女使用地屈孕酮治疗前后的血清中Th1型细胞因子(IL-2和TNF-α)及Th2型细胞因子(IL-4和IL-10)的表达水平。结果先兆流产组治疗后血清中IL-2和TNF-α的表达水平与治疗前相比明显降低(P值均<0.05),与正常妊娠组相比差异无统计学意义(P值均>0.05);而先兆流产组血清中IL-4和IL-10的表达水平在治疗前后差异无统计学意义(P值均>0.05),治疗后略有上升,达到和正常妊娠组相当的水平(P值均>0.05)。结论孕激素治疗先兆流产的机制可能在于促进Th2型细胞因子的分泌,纠正Th1/Th2型细胞因子的病理偏移,使其向着正常妊娠时的平衡移动,即由Th1向Th2的移动。Objective To investigate the effects of progestogen dydrogesterone on Th1/Th2 cytokines balance in the serum of women with threatened abortion,and elucidate the mechanisms that underlie the treatrment of threatened abortion through the use of dydrogesterone.Methods The concentrations of Th1 cytokines（IL-2 and TNF-α）and Th2 cytokines（IL-4 and IL-10）in the serum of 42 normal pregnant women and 60 women with threatened abortion before and after the treatment with dydrogesterone were measured by enzyme-linked immunosorbant assay（ELISA）.Results The concentrations of IL-2 and TNF-α in the serum of threatened abortion post-treatment group were significantly lower than those in threatened abortion pre-treatment group（P0.05）,and compared with those in normal pregnancy group（P0.05）.While the concentrations of IL-4 and IL-10 in the serum of threatened abortion post-treatment group were slightly higher than those in threatened abortion pre-treatment group（P0.05）,and compared with those in normal pregnancy group（P0.05）.Conclusion The mechanism of treatrment of threatened abortion through the use of progestogen might be through promoting secretion of Th2 cytokines,recovering the pathological shift of Th1/Th2 cytokines balance,and making the balance of Th1/Th2 cytokines deviate to Th2 cytikines reaction,i.e.the state of normal pregnancy.
