喹诺酮类药合理应用评估与干预研究

摘　　要：目的:探讨系统化处方评估与干预工作模式对促进喹诺酮类药临床合理应用的效果。方法:运用系统化处方评估与干预工作模式,组建专家小组指导建立评估标准,制定药师发药前处方审核规范,综合制定医师合理用药干预策略,并抽取我院门诊2010年1月(干预前)和2011年1月(干预后)全部喹诺酮类药处方,分别为2029张及1961张,进行合理性评估及干预前、后对照研究。结果:干预后我院门诊喹诺酮类药应用率降低,非适应证用药由干预前的9.46%降至干预后的3.93%(P<0.01),禁忌证用药由干预前的0.34%降至0(P<0.01),用法用量不适宜由干预前的6.31%降至2.04%(P<0.01),联合用药不适宜由干预前的12.17%降至6.68%(P<0.01)。结论:系统化处方评估与干预工作模式对于促进我院喹诺酮类药临床合理应用的效果显著。OBJECTIVE： To investigate the effect of systematic prescription evaluation and intervention model on improving rational use of quinolones. METHODS： The systematic prescription evaluation and intervention model were put into practice which included establishing the expert evaluation group to develop the evaluation standards of rational use of quinolones, formulating the prescription checking standards before pharmacist dispensing drugs and making comprehensive intervention measures. All outpatient quinolone prescriptions in Jan. 2010 （before intervention） and Jan. 2011 （after intervention） were sampled for rationality evaluation and contrast study before and after intervention. RESULTS： The utilization ratio of quinolone in outpatient department of our hospi- tal decreased after intervention, the drug use of non-indications was reduced from 9.46% to 3.93% （P〈0.01） and drug use of contraindication was decreased from 0.34% to 0（P〈0.01）, the occurrence of incorrect dosage and usage was reduced from 6.31% to 2.04% （P〈0.01）, and the occurrence of inappropriate drug combination was reduced from 12.17% to 6.68% （P〈0.01）. CONCLUSION： Systematic prescription evaluation and intervention model can significantly improve rational use of quinolone in our hospital.
