摘  要:[目的]探讨血、尿β2-微球蛋白(β2-MG)对糖尿病肾病早期肾功能损害的变化和意义。[方法]采用免疫比浊法对早期糖尿病肾病患者(早期DN组)、单纯糖尿病者(DM组)及健康体检者(正常对照组)的血、尿β2-MG含量水平进行检测。同时对3组研究对象进行血尿素氮(BUN)、血肌酐(Cr)和24 h尿白蛋白的定量测定。[结果]早期DN组血、尿β2-MG浓度明显高于DM组,DM组血、尿β2-MG浓度明显高于正常对照组,组间差异均有统计学意义(P﹤0.05)。早期DN组24 h尿白蛋白的量显著高于单纯DM组和对照组(P﹤0.05),而单纯DM组和正常对照组差异无统计学意义(P﹥0.05)。血BUN、血Cr在正常对照组、早期DN组和单纯DM组中比较差异均无统计学意义(P﹥0.05)。[结论血、尿β2-MG是早期肾脏损害的灵敏指标,对于糖尿病患者应定期检查血、尿β2-MG的变化,以便及时发现早期肾损害,有效预防慢性肾功能衰竭的发生。[Objective]To explore the changes and significance of blood and urine β2-microglobulin(β2-MG)in impaired renal function of diabetic nephropathy.[Methods]Blood and urine β2-MG concentration levels were detected for early diabetic nephropathy(early DN group),diabetes(DM group)and healthy individuals(control group)using immunoturbidimetry.Blood urea nitrogen(BUN),creatinine(Cr)and 24-hour urinary albumin in subjects of three groups were detected at the same time.[Results]Blood and urine β2-microglobulin level in early DN group and DM group were respectively significantly higher than that in DM group and control group(P﹤0.05).24-hour urinary albumin contents in early DN group were significantly higher than those of DM group and control group(P﹤0.05),while there were no significant differences between the levels of BUN and blood Cr in the three groups(P﹥0.05).[Conclusion]Blood and urine β2-MG are sensitive indicators for early kidney damage,so blood and urine β2-MG should be regularly checked for diabetic patients in order to find early kidney damage,and prevent the occurrence of chronic renal failure.
