摘 要:目的探讨地塞米松(DXM)对热性惊厥(FS)患儿外周血单个核细胞(PBMC)细胞间黏附分子-1(ICAM-1)及配体淋巴细胞功能相关抗原-1(LFA-1)表达和分泌的影响。方法 FS组患儿16例分为FS对照组和DXM干预组,DXM干预组采用1.0μmol.L-1的DXM进行干预;健康对照组16例,为年龄和性别与FS组患儿相匹配的同期体检健康儿童。常规方法进行FS组和健康对照组的PBMC培养。采用流式细胞仪检测PBMC表面ICAM-1、LFA-1表达水平;采用ELISA法检测培养上清中ICAM-1、LFA-1水平。结果 FS对照组在体外培养的PBMC表面ICAM-1、LFA-1表达水平分别为(19.78±8.93)%和(43.05±15.85)%,明显高于健康对照组[(13.88±6.73)%,(30.43±16.25)%](Pa<0.05);DXM干预组的PBMC表面ICAM-1、LFA-1表达水平分别为(17.14±7.98)%和(33.93±19.76)%,与FS对照组比较有下调趋势,但差异无统计学意义(Pa>0.05)。FS对照组PBMC的ICAM-1、LFA-1分泌水平分别为(6.73±3.88)%和(4.84±2.95)%,明显高于健康对照组[(4.24±2.42)%,(2.76±1.36)%](Pa<0.05);DXM干预组PBMC的ICAM-1、LFA-1分泌水平[(4.34±1.82)%,(3.06±2.81)%]明显低于FS对照组(Pa<0.05)。结论糖皮质激素DXM干预后ICAM-1、LFA-1的表达被抑制,抑制FS患儿ICAM-1、LFA-1的高表达可作为其治疗的新靶点。Objective To analyze the expressions of peripheral blood mononuclear cells (PBMC) intercellular adhesion molecule - 1 ( ICAM - 1 ) and lymphocyte function - associated antigen - 1 ( LFA - 1 ) immunoreaction in patients following febrile seizures(FS) , in order to find out the neuroimmunomodulation mechanisms in the pathogenesis of FS. Methods Sixteen cases of FS were divided into 2 groups randomly:FS control group and dexamethasone (DXM) - treated group. Sixteen cases of healthy children whose age and sex were matched to the patients were selected as healthy control group. PBMC were dissected, purified and cultured in conventional method. The expression levels of PBMC ICAM - 1 and LFA - 1 were detected by flow eytometry. Then, ICAM - 1 and LFA - 1 concentration in these supernatants were detec- ted with enzyme-linked immunosorbent assay. Results ICAM-1 and LFA-1 expressions [(19.78±8.93)% and (43.05±15.85)%] in PBMC cultured in vitro in FS control group were significantly higher than those in normal control group [ (13.88± 6.73 )% and (30.43 ± 16.25 ) % ] ( Pa 〈 0.05 ). Compared with FS control group, ICAM - 1, LFA - 1 expressions [ ( 17.14 -± 7.98 ) % and (33.93 ± 19.76 ) ] in DXM group represented downtrend after the treatment with DXM in PBMC, but there was no statistical difference between the 2 groups ( Pa 〉 0.05 ). The secreting values of ICAM - 1 and LFA - 1 expressions in PBMC cultured in vitro in FS control group were (6.73 ± 3.88) % and (4.84 ±2.95 )%, which were significantly higher than the secreting levels in normal control group [ (4.24 ± 2.42 )% and (2.76 ± 1.36) % ]. On the other hand, ICAM - 1 and LFA - 1 secretion levels [ (4.34 ± 1.82) % and (3.06 ± 2.81 ) % ] were significantly lower than those in FS control group, and there was statistical difference between the 2 groups (P 〈 0.05 ).
