透明质酸在妇产科腹盆腔手术术后粘连预防中的临床效果观察

摘　　要：目的:探讨透明质酸(HA)在妇产科腹盆腔手术术后粘连预防中的临床效果。方法:选取2008年8月～2011年7月的500例妇产科腹盆腔手术患者为研究对象,将其随机分为对照组和观察组各250例,观察组在对照组的基础上加用HA进行术后粘连预防,后将两组患者的术后粘连发生率及手术前后的血清IL-6、IL-10、TNF-α、CRP水平、血液流变学指标进行统计及比较。结果:观察组中初次手术和再次手术患者的粘连发生率及重度粘连发生率均低于对照组,血清IL-6、IL-10、TNF-α、CRP水平低于对照组,血液流变学指标也低于对照组,差异有统计学意义(P<0.05或P<0.01)。结论:HA在妇产科腹盆腔手术术后粘连预防中的临床效果明显,可显著降低粘连发生率及改善多项相关血清因子。Objective：To study the clinical effect of Hyaluronan（HA） in the prevention of adhesions after abdominal and pelvic surgery in department of obstetrics and gynecology.Methods：500 patients treated with abdominal and pelvic surgery from August 2008 to July 2011 in department of obstetrics and gynecology were selected as research object,and they were randomly divided into control group and observation group,with 250 patients in each group,and the observation group was given preventive treatment with HA based on the control group,then the rates of adhesions and severe adhesions,serum IL-6,IL-10,TNF-α,CRP,hemorheology indexes before and after the treatment of two groups were analyzed and compared.Results：The rates of adhesions and severe adhesions of first operation and reoperation of observation group were all lower than those of control group,serum IL-6,IL-10,TNF-α,CRP were all lower than those of control group,hemorheology indexes were all lower than those of control group,there were all significant differences（P〈0.05 or P〈0.01）.Conclusion：The clinical effect of HA in the prevention of adhesions after abdominal and pelvic surgery in department of obstetrics and gynecology is obvious,and it can significantly decrease the rate of adhesions and improve multiple items related serum factors.
