摘 要:目的观察压力超负荷大鼠心肌细胞CD4、CD8抗原及肿瘤坏死因子(TNF-α)的表达水平,探讨免疫反应在促心肌肥厚中的作用,以及他汀类药物阻止心肌肥厚发生可能的作用机制。方法 SD大鼠30只,随机分为对照组、心肌肥厚组、脾细胞输注组、心肌肥厚+他汀组、脾细胞+他汀组,通过缩窄腹主动脉制造心肌肥厚模型,将心肌肥厚大鼠的脾细胞输注给正常SD大鼠,以超声指标观察脾细胞的致心肌肥厚作用,以免疫组化法观察CD4、CD8抗原及炎症因子TNF-α的表达。并用他汀类药物进行干预,重复观察上述指标。结果正常SD大鼠输注心肌肥厚大鼠的脾细胞后,心肌发生一定程度的肥厚,超声指标较输注前升高;除对照组外,其他各组心肌上的CD4,TNF-α蛋白表达均升高,而CD8蛋白表达下降;心肌肥厚组和脾细胞输注组用他汀类药物干预后,其超声指标及CD4,TNF-α蛋白表达均有不同程度的下降,而CD8蛋白表达上升。结论免疫反应参与了大鼠心肌肥厚的发生及发展;他汀类药物可能通过调节免疫抑制心肌肥厚的发生。Objective To observe the level of CD4, CDs and tumor necrosis factor -α(TNF - α) in the cadiocytes of pressure - overload rats, study inflammatory reaction in the process of myo- cardial hypertrophy and investigate the mechanism that the anti - hypertroyphic effects of statins through immune and inflammatory regulation. Methods Thirty SD rats were randomly divided into five groups: control group, myocardial hypertrophy group, spleen cell infusion group, myocardial hypertro- phy plus statin -treatment group, spleen cell infusion plus statin -treatment group. Myocardial hyper- trophy model was made via narrowing of the abdominal aorta, then spleen cells of myocardial hyper- trophic rats were infused into normal rats, and subsequent observation of pro - hypertrophic effects in myocardium of affected rats were made via echocardiogram. In the meantime, the expression of CD4, CDs antigens and inflammatory factor TNF - αin the myocardial cells of all five groups before and after statins treatments were studied and compared. Results Splenic cell infusion of rats caused certain de- gree of myocardial thickening, except the control group. There was a tendency of increase of CD4, TNF -α and decrease of CDs in the other four groups of rats, but statin treatments reversed that tend- ency. Conclusion Immune and inflammatory reaction could be responsible for the genesis and devel- opment of myocardial hypertrophy in rats. Statins may regulate immune and inflammatory reaction and suppress the myocardial hypertrophic course of progression.
