舒芬太尼鞘内注射抑制阑尾牵拉反应的临床观察

摘　　要：目的探讨舒芬太尼复合布比卡因鞘内注射在阑尾切除术中对内脏牵拉反应抑制的临床意义。方法选择ASAI～Ⅱ级在腰硬联合阻滞下行阑尾切除术患者40例，随机分为2组：对照组（n=20），0．75％布比卡因15mg；观察组（n=20），0．75％布比卡因15mg＋舒芬太尼5μg（0．5mL），对术中牵拉反应、镇静2个方面进行评估。结果2组在年龄、体重等比较差异均无统计学意义，在抑制内脏牵拉反应、镇静方面，观察组明显均优于对照组。结论舒芬太尼复合布比卡因鞘内注射对阑尾牵拉反应有明显的抑制作用。Objective To investigate the clinical significance of Sufentanil combined with Bupivacaine intrathecal in appendectomy surgery in inhibitionof the visceral reaction. Methods Forty cases of ASA Ⅰ-Ⅱ Level in the epidural block down appendectomy surgery were randomly divided into 2 groups： control group （n = 20）, 0.75% Bupivacaine 15 rng; the observation group （ n = 20 ）, 0.75 % Bupivacaine 15 mg ＋ Sufentanil 5 μg （0.5 mL）. Patients＇ response to traction and sedation were assessed . Results There was no significant difference in age and weight in 2 groups. In the inhibition of visceral reactions and sedation, the observation group was better than the control group. Conclusion Intrathecal injection of Bupivacaine combined with Sufentanil can significantly inhibit the response of appendix stretch.
