摘  要:目的 利用多普勒超声心动图(Doppler UCG) 运动负荷试验检测正常老年和老年冠心病(CHD) 患者在次极量运动负荷下收缩和舒张功能的储备能力。方法 对89 例(正常青年组20 例,正常老年组31 例,老年CHD 组38 例)进行次极量卧位踏车,同步做心电图和Doppler UCG 运动试验及99m TcMIBI运动心肌显像试验。采用Doppler 超声技术记录运动前、后主动脉和二尖瓣血流频谱。结果 负荷条件下,正常老年组收缩功能各参数增加幅度的百分比明显低于正常青年组,但明显高于老年冠心病组;运动后青年组以A 峰增加为主,正常老年组以E 峰增加为主,老年CHD 组E峰和A峰增加幅度的百分比明显低于正常老年组。结论 次极量负荷下,收缩储备功能健康老年人较年轻人降低,老年CHD患者更下降;健康老年人和老年CHD 患者运动后可能心脏舒张储备和左房代偿功能下降。Purpose To investigate left ventricular systolic and diastolic reserved function of normal aged and aged subjects with coronary heart disease (CHD) under submaximal stress conditions. Methods 89 subjects (young healthy 20,aged healthy 31,aged CHD 38) performed submaximal supine bicycle exercise with electrocardiography,ultrasonic cardiography and 99m Technetium imaging exercise testings done synchronously.The aortic and mitral flow patterns were recorded by Doppler echocardiography. Results The percent changes in aortic variables of systolic function of aged healthy subjects were much lower than those of young healthy subjects,and much higher than those of aged CHD patients.After exercise,the A/E ratio increased in young group,while decreased in aged healthy and CHD groups. Conclusion Under submaximal stress conditions,the cardiac systolic reserved function was obviously decreased in aged healthy subjects,and much lower in aged CHD patients.The changes of left ventricular filling patterns in healthy aged and aged CHD groups might suggest an elevation in left atrial pressure and decreased cardiac diastolic reserved and left atrial compensatory functions.
