主观性耳鸣患者144例药物治疗效果及影响因素分析

摘　　要：目的探讨影响耳鸣药物治疗的相关因素,为临床治疗提供可参考的适宜药物治疗方案。方法应用银杏制剂联合甲钴胺对144例以主观性耳鸣为首发症状的患者进行治疗,对影响治疗效果的6项指标,包括年龄、病程、健康状况(并发症)、匹配响度、匹配频率与纯音听阈升高最高频率的一致性、听力改善进行多因素非条件logistic回归分析。结果 144例患者中,治愈37例,有效69例,无效38例,总有效率73.6%。经多因素logistic回归分析,显示对该药物疗效影响的相关因素依次为:听力改善(OR=2.829,P<0.01)、病程(OR=0.307,P<0.01)。结论银杏制剂联合甲钴胺治疗主观性耳鸣有效,听力改善的程度越好,病程越短,则治疗效果越理想。Objective To evaluate the related factors influencing efficacy of drug treatment on patients with tinnitus so as to provide clinical reference for suitable scheme of drug treatment. Methods A total of 144 patients with premier symptom of tinnitus were treated with ginaton and mecobalamine. Multiple logistic regression analysis was performed with 6 factors affecting curative effect [ age, course of disease, health condition （complication）, matching volume of tinnitus, conformance between the matching frequency and hearing lose in the highest frequency, improvement of audition ] as independent variables and with therapeutic effect as dependent variable. Results Of 144 patients, 37 cases were cured, and 69 cases were get better, and 38 cases were not cured. The total effective rate was 73.6%. The result of multiple logistic regression analysis revealed that the relative factors influencing efficacy of drug therapy were improvement of audition（ OR = 2.829 ,P 〈 0. 01 ）, course of disease （ OR = 0.307,P 〈 0. 01 ）. Conclusions Treating tinnitus with ginaton and mecobalamine is more effective if the patient has more improvement in audition or has a briefer course of tinnitus.
