多巴酚丁胺心内膜自动检测技术对左室功能的研究

摘　　要：目的　探讨多巴酚丁胺心内膜自动检测技术（ＡＢＤ） 测量左室功能的临床应用价值。方法　应用低剂量多巴酚丁胺心内膜超声自动检测技术对２３ 例二尖瓣病变患者和２７ 例心肌病患者的左室收缩和舒张功能进行了研究。结果　ＡＢＤ技术和二维超声测量的左室容积和射血分数无显著性差异（Ｐ均＞０－０５）；ＡＢＤ 技术测量的Ａ组患者的左室收缩和舒张功能指标均低于Ｂ组患者（Ｐ＜ ０－０５）。应用多巴酚丁胺后，两组ＥＦ、ＰＦＲ、ｄｖ／ｄｔ 均显著增加；Ｂ组ＲＦＳ和ＥＤＦＶ／ＬＤＦＶ亦明显增加。结论　多巴酚丁胺心内膜自动检测技术可简便。Purpose To explore the value of clinical application of dobutamine automatic border detection for measuring left ventricular function. Methods The left ventricular systolic and diastolic functions were studied by low dose dobutamine echocardiography with automatic border detection(ABD) technique in 23 cases with mitral valvular diseases(group A)and 27 cases with cardiomyopathy(group B). Results There were no significant difference in the left ventricular systolic or diastolic volume indexes,as well as ejection fractions between ABD and two dimensional echocardiography( P >0 05).The parameters of left ventricular volume derived by ABD in group A were decreased than those in group B( P <0 05).After low dose dobutamine infusion,EF peak filling rate(PFR),dv/dt were significantly increased in both groups( P <0 05),and rapid filling slop(RFS)and EDFV/LDFV were also increased in group B( P <0 05). Conclusion Dobutamine ABD technique can provide simple and accurate method for the estimation of left ventricular systolic and diastolic functions. [
