NAH个体化方案治疗高血压技术的卫生经济学分析

摘　　要：目的将NAH个体化方案治疗高血压技术与基层常用高血压治疗技术进行比较,研究NAH个体化方案治疗高血压技术推广的社会效益。方法以宁夏回族自治区青铜峡市的三所乡镇卫生院作为实施现场,将182例原发性高血压患者随机分为适宜技术组与常规技术组,每组各91例。适宜技术组接受NAH个体化高血压治疗方案,常规技术组接受基层最常用的高血压治疗技术,比较NAH个体化方案治疗高血压技术与基层常用高血压治疗技术的效果与费用情况。结果治疗2周后,采用NAH个体化治疗方案的适宜技术组高血压患者的降压控制率高于常规技术组;且患者每日药物费用平均为(0.13±0.15)元,低于常规技术组的(0.50±1.27)元。采用NAH个体化治疗方案后,平均每年可以为每位患者节省135.05元的医疗费用。结论 NAH个体化方案治疗高血压技术的推广在项目地区取得了较好的社会效益,应采取积极的措施促进该项适宜技术的进一步推广。Objective To investigate the social benefits of promoting the NAH treatment program through comparison between individual treatment with Nifedipine＋Atenolol＋Hydrochlorothiazide（NAH） and conventional treatment for hypertension. Methods Three rural hospitals from Qingtongxia City of Ningxia Hui Autonomous Region were chosen as experimental construction sites.and 182 hypertensive patients were divided randomly into 2 groups of appropriate technique and conventional technique,91 in each.the patients in the appropriate group were given NAH treatment,while those in the conventional group were given the treatment most commonly used at the grass roots.The effectiveness and cost were compared between the two groups. Results After 2-week treatment,blood pressure control rates of appropriate group was higher than that of conventional group,while the daily drug cost [RMB（0.13±0.15） yuan] was lower than that [RMB（0.50±1.27） yuan],and the average saved coast for each patent per year was RMB 135.05 yuan. Conclusion NAH treatment has achieved better social benefits in the pilot regions.and the positive actions should be taken for promoting further popularization of the program.
