摘　　要：[目的 ]　探讨遗传因素与环境因素在Ⅱ型糖尿病病因组合中的相互关系 ,比较两者对Ⅱ型糖尿病作用的大小。 [方法 ]　在湖南长沙市 4所医院中 ,按 1∶1配对 (同性别 ,对照与病例年龄相差不超过 5岁 )收集同期诊治的Ⅱ型糖尿病和非Ⅱ型糖尿病 2 0 0对 ,资料采用条件Logistic回归分析。[结果 ]　Ⅱ型糖尿病的危险因素有 :糖尿病家族史 (Bi =4 195 ,Bs =2 5 2 3 ,OR =66 3 5 4) ,肥胖 (Bi =2 184,Bs =1 93 2 ,OR =8 882 ) ,肥肉 (Bi =1 2 0 4,Bs =0 814 ,OR =3 3 3 3 ) ,暴饮暴食 (Bi =1 461,Bs =0 75 6,OR =4 3 10 )和甜食 (Bi=0 85 2 ,Bs =0 5 67,OR =2 3 4 4)。 [结论 ]　在Ⅱ型糖尿病的病因组合中 ,遗传因素是最重要的危险因素 ,其次是肥胖、肥肉、暴饮暴食和甜食。但是 ,这些危险因素对导致人群Ⅱ型糖尿病的社会效应以肥胖为最大 ,其次是糖尿病家族史、暴饮暴食、甜食和肥肉。Objective] To study the relationship between environment and genetic factors in combination of pathogeny and conpare both effect for NIDDM. [Methods] The 400 Subjects from four hospitals in Hunan Changsha city were selected in the study. The 200 cases (Non-Insulin-Dependent Diabatees Mellitus, NIDDM) and 200 age-sex-matched controls (same sex, age difference less than 5 years) were diagnosed according to 'WHO diabetes mellitus criteria' (1985). Conditional Logistic regression analysis was used for data analysis. [Result] five possible risk factors were 1) family history of diabetes (Bi=4.195, Bs=2.523, OR=66.354), 2) obesity (Bi=2.184, Bs=1.932, OR=8.882), 3)fat (Bi=1.204, Bs=0.814, OR=3.333), 4) over eating and drinking (Bi=1.461, Bs=0.756, OR=4.310), 5) more consumption of sugar (Bi=0.852, Bs=0.567, OR=2.344). [Conclusion] Genetic factor is the most important among the risk factors of NIDDM, next is obesity, and then fat, over eating and drinking, more consumption of sugar. However, as for the social effect of these risk factors, obesity is the first risk factor, sccondly, genetic factor, then over eating and drinking, more consumption of sugar, fat.
