GDM孕妇血脂联素水平与糖脂代谢的相关性研究

摘　　要：目的:了解妊娠期糖尿病(GDM)患者血清脂联素(APN)水平与糖脂代谢的关系,探讨APN是否参与GDM孕妇糖脂代谢紊乱的形成。方法:采用ELISA法测定30例妊娠期糖尿病孕妇和30例正常糖耐量孕妇(NGT)空腹血清APN水平;同时测定两组孕妇空腹血清总胆固醇(TCH)、甘油三酯(TG)、极低密度脂蛋白胆固醇(VLDL-C),空腹血糖(FPG)及糖化血红蛋白(HbA1c)水平。结果:①GDM组孕妇血清APN水平明显低于NGT组,差异有统计学意义(P<0.05)。②GDM组孕妇血TCH、TG、VLDL-C水平明显高于NGT组孕妇(P<0.05)。③GDM组孕妇FPG、HbA1c水平明显高于NGT组孕妇(P<0.01)。④相关分析显示,两组孕妇血清APN与孕晚期BMI、FPG、HbA1c、TCH、TG及VLDL-C均存在负相关关系(P<0.01或P<0.05),与孕前BMI无明显相关性(P>0.05)。结论:GDM患者血APN水平明显降低,APN水平降低可能参与诱导GDM患者糖脂代谢紊乱的形成。Objective： To understand the relationship between serum adiponectin （APN） level and glucolipid metabolism in pregnant women with gestational diabetes mellitus （ GDM）, explore whether APN participates in the formation of glucolipid metabolic disorders a- mong the pregnant women with GDM. Methods： Enzyme -linked immunosorbent assay （ELISA） was used to detect the fasting serum APN levels in 30 pregnant women with GDM and 30 pregnant women with normal glucose tolerance （NGT） ; the levels of fasting serum total cholesterol （TCH）, triglyceride （TG）, very low -density lipoprotein cholesterol （VLDL -C）, fasting plasma glucose （FPG）, and glycosylated hemoglobin Alc （HbAlc） in the two groups were also measured at the same time. Results： ①The fasting serum APN level in GDM group was significantly lower than that in NGT group （P 〈0. 05） . ②The levels of fasting serum TCH, TG, and VLDL- C in GDM group were significantly higher than those in NGT group （ P 〈 O. 05 ） . ③The levels of FPG and HbAlc in GDM group were significantly higher than those in NGT group （ P 〈 0. 01 ） . ④The results of correlation analysis showed that in the two groups, there was negative correlation between serum APN levels and BMI during the third trimester of pregnancy, FPG, HbAlc, TCH, TG, and VLDL - C （P 〈0. 01 or P 〈0. 05）, but there was no correlation between serum APN levels and BMI before pregnancy in the two groups （P 〉 0. 05） . Conclusion： The serum APN levels in pregnant women with GDM decrease significantly, which may participate in the induction of formation of glucolipid metabolic disorders among the pregnant women with GDM.
