手术治疗股骨粗隆间骨折时对小粗隆的处理

摘　　要：目的探讨Ⅳ型股骨粗隆间骨折手术治疗时对小粗隆骨折固定与否的治疗效果。方法用股骨近端锁定钢板固定治疗新鲜Ⅳ型股骨粗隆间骨折78例,其中小粗隆骨折固定（以下称为A组）治疗36例,小粗隆骨折不固定（以下称为B组）治疗42例,随访8~36个月（平均19个月）。结果两组患者中手术时间及术中出血量比较有统计学意义（P〈0.05）,而骨折愈合时间及髋关节功能无统计学意义（P〉0.05）。结论Ⅳ型股骨粗隆间骨折小粗隆骨折固定与不固定,在骨折愈合及髋关节功能方面无统计学意义,但小粗隆骨折不固定组在术中出血及手术时间上明显优于固定组。Objective To investigate the type Ⅳ intertrochanteric fracture of femur with surgical fixation of lesser trochanter fracture and the need for treatment.Methods The proximal femoral locking plate fixation of fresh Ⅳ intertrochanteric fractures of femur in 78 cases,including lesser trochanter fractures fixed（here in after referred to as A group） treatment of 36 cases,lesser trochanter fractures unfixation（here in after referred to as B group） treatment of 42 cases,followed up for 8 to 36 months（mean 19 months）.Results Both patients compared operative time and blood loss have statistical significance（P0.05）,but union of fracture time and articulation of hip function have no significant difference（P0.05）.Conclusion Type Ⅳ intertrochanteric fracture of femur fix lesser trochanter fractures or not in fracture healing and joint function,the two treatments have no significant difference.But the lesser trochanter fracture unfixation in blood loss and operation time is obviously better than the fixed group.
