围绝经期妇女肺栓塞临床特点

摘　　要：目的探讨围绝经期妇女肺血栓栓塞症(PTE)的特点。方法将已行CT肺动脉血管造影(CTPA)的围绝经期妇女分为PTE组30例,非PTE组(对照组)25例。比较两组妇女的危险因素、临床特点、体质量指数、血脂及凝血四项的差异。结果 PTE组呼吸困难26例(86.7%)、胸闷11例(36.7%)、胸痛10例(33.3%);常见体征为P2亢进16例((53.3%)、双下肢不对称性水肿10例(33.3%)、紫绀10例(33.3%)。PTE组呼吸困难、P2亢进、紫绀、双下肢不对称性水肿发生率均明显高于非PTE组(P均<0.05)。两组血脂水平均高,PTE组中肥胖的比例高于非PTE组(P<0.05)。结论围绝经期妇女脂代谢失调,肥胖的围绝经期妇女出现呼吸困难、P2亢进、紫绀、双下肢不对称性水肿时,应警惕PTE的存在。Objective To explore the clinical characteristics of pulmonary thromboembolism （PTE） in perimenopausal women. Methods Perimenopause women done computer tomography of pulmonary artery angiography （ CTPA ） test were divided into two groups, PTE group （30 cases） and non-PTE group （25 cases）. To compare the differences of risk factor, clinical characteristics, body mass index （BMI）, blood lipids and four items of blood coagulation between two groups. Results In PTE group, 26 cases （86.7%） with dyspnea symptom, 11 cases （36.7%） with chest tightness, 10 cases （33.3%） with chest pain, 16 cases （53.3%） with hyperfunction of pulmonic second sound, 10 cases （33.3%） with double lower limb asymmetry edema, and 10 cases （33.3%） with cyanosis. The incidence rate of dyspnea, hyperfunction of pulmonic second sound, cyanosis, double lower limb asymmetry edema in PTE group were significantly higher than those in non-PTE group （ P 〈 0.05 ）. The blood lipids levels were high in two groups. The proportion of obesity in PTE group was significantly higher than that in non-PTE group （ P 〈 0.05）. Conclusions The lipid metabolism disorder exists in perimenopause women. When dyspnea, hyperfunction of pulmonic second sound, cyanosis, double lower limb asymmetry edema are found in obese perimenopausal women, we should be alert to the occurrence of PTE.
