正常糖耐量人群血浆网膜素水平及其影响因素

摘　　要：[目的]探讨正常糖耐量人群血浆网膜素水平以及与肥胖、血糖脂水平、胰岛素分泌和作用等因素的关系。[方法]随机选择健康体检者90名,其中正常体重者63名(33名男性,30名女性),超重/肥胖者27名(22名男性,5名女性)。采用ELASA(酶联免疫分析)法测定血浆网膜素水平。分析网膜素与空腹血脂谱、体质指数(BMI)、空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR)之间的关系。[结果]①超重/肥胖者较正常体重者血浆网膜素水平为低,两组之间比较差别具有显著性(P<0.05)。②校正年龄、性别后血浆网膜素水平与BMI、HOMA-IR及腰围呈负相关。③校正年龄、性别后血浆网膜素水平与HDL呈正相关。④多元逐步回归分析显示,BMI及HOMR-IR是影响血浆网膜素水平的显著因素。[结论]①血浆网膜素水平与胰岛素敏感性呈正相关。②肥胖可能负性调节网膜素基因的表达,或阻止其释放入血。[ Objective] To observe blood plasm omentin level in normal glucose tolerance people and its relevance to obsesity,insulin secretion and blood lipid level. [ Methods] Ninety healthy people who underwent physical examination were allocated into two groups, in which sixty-three people had normal weight （ thirty-three men and thirty women） and twenty seven people （twenty-two men and five women） had over weight or obesity. ELASA method was adopted to analyze the blood omentin level. And the relationship between the omentin level with fasting level of lipid, BMI,nsulin level and HOMA-IR were assessed. [ Results] The blood omentin level was lower in over weight or obesity people than that in normal weight people （ P 〈 0.05 ）; The blood omentin level was negative in correlation to BMI, HOMA-IR and waistline （ adjusting age, sex） ;The blood omentin level was positive in correlation to HDL（ adjusting age, sex ） ; Multiple linear stepwise regression analysis revealed BMI and HOMR-IR were distinct interfering fctors to omentin. [ Conclusion] The blood omentin level is positively correlated to insulin sensitivity. Obesity maybe negatively regulates the expression of omentin gene or prevent it from being released to blood.
