补肾活血法治疗慢性心力衰竭的临床研究

摘　　要：目的观察补肾活血法对慢性心力衰竭的临床疗效。方法将128例慢性心力衰竭患者随机分为治疗组和对照组,各64例。对照组给予常规抗心力衰竭治疗(血管紧张素受体拮抗剂、β受体阻滞剂、血管扩张剂、利尿剂、强心剂等药物),治疗组在对照组治疗基础上加用补肾活血为主的补肾活血汤,疗程4周。观察慢性心力衰竭患者治疗前后中医临床症状、体征、纽约心脏病协会(NYHA)心功能分级,应用彩色多普勒测定左室射血分数(LVEF)、左室舒张末内径(LVEDD),测定血清脑钠肽(BNP)浓度、行6min步行试验(6MWT)。结果治疗组总有效率95.3%,优于对照组的78.1%(P<0.05)。两组治疗后LVEF、LVEDD、BNP、6min步行试验结果均有改善,治疗组优于对照组(P<0.05)。结论补肾活血法对慢性心力衰竭具有确切的疗效,对心功能各项指标均具有明显的改善作用。Objective To observe the clinical effect of reinforcing kidney and activating blood circulation therapy（Bushen Huoxue decoction） on the chronic heart failure（CHF）.Methods One hundred and eight patients（pts） with CHF were randomly divided into two groups：Control group（n=64） treated by routine treatment（angiotension receptor blocker,β-receptor blocker,vasodilator,diuretic） and treatment group（n=64） treated by basic routine and Bushen Huoxue decoction for 4 weeks.Clinical symptoms,signs and New York Heart Association（NYHA） heart classification were studied before and after treatment.The data of left ventricular ejection fraction（LVEF）,left ventricular end diastolicdiameter（LVEDD）,serum brain natriuretic peptide（BNP）,and 6 minutes walk test（6MWT） were collected.Results The total clinical effective rate in treatment group was 95.3%,which was significantly higher than that in control group（78.1%,P〈0.05）.The LVEF,LVEDD,BNP,and 6MWT in both groups were improved following the treatment.And that in treatment group was superior to that in control group（P〈0.05）.Conclusion The reinforcing kidney and activating blood circulation therapy was effective for chronic heart failure and could obviously improve the heart function.
