高血压病痰瘀互结证患者血管彩超的动态临床研究

摘　　要：目的观察高血压病痰瘀互结证患者经方证对应汤剂治疗后其颈动脉内膜、心脏功能及结构血管超声的动态变化,以揭示其发生发展规律及动脉硬化的相关性。方法根据舌诊、脉诊及主症辨证共入组82例患者,其中给予70例痰瘀互结证患者祛痰化瘀、解毒调络方加减治疗,连续3个疗程后观察患者症状、血压变化及心脏、颈动脉血管形态变化。结果 3个疗程后,高血压病痰瘀互结患者在颈动脉超声检测中异常者由36例(占51.43%)降至33例(占47.14%),在心脏检测中异常者由30例(占42.86%)降至29例(占41.43%)。结论高血压病痰瘀互结证患者经痰瘀同治法干预后,可以明显改善患者的症状,提高心脏舒张功能,减少动脉内—中膜增厚、粥样斑块形成,对于兼夹颈动脉粥样硬化早期表现患者较为有效。Objective To observe the dynamic changes in carotid endarterium,heart function and vascular structure in ultrasonic test in the treatment of hypertension of the interaction of phlegm and stasis pattern/syndrome according to pattern/syndrome differentiation so as to display the correlation between the development rule of the disease and arteriosclerosis.Methods According to the tongue and pulse diagnosis as well as the differentiation of main symptoms,82 cases were included.Of them,70 cases of the interaction of phlegm and stasis pattern/syndrome were treated with the modified formula in light of resolving phlegm and stasis,detoxification and regulating vessels.After continuous 3-session treatment,the symptoms,blood pressure and morphological changes in the heart and carotid artery were observed.Results After the treatment for 3 sessions,of the patients with hypertension of the interaction of phlegm and stasis pattern/syndrome,in carotid artery ultrasonic test,the number of abnormality was reduced to 33 cases（47.14%）from 36 cases（51.43%）.Concerning to the heart test,the number of abnormality was reduced to 29 cases（41.43%）from 30 cases（42.86%）.Conclusion The intervention of hypertension of the interaction of phlegm and stasis pattern/syndrome improves apparently in the symptoms,increases diastolic function of the heart and reduces the intima-medial thickness of carotid artery and the formation of atheromatous plague on the basis of the simultaneous removal of phlegm and stasis.This therapy is effective for the patients combined with the early-stage manifestation of carotid atherosclerosis.
