月经期应用解毒化浊中药治疗慢性输卵管炎性不孕症50例临床观察

摘　　要：目的观察月经期应用解毒化浊中药治疗慢性输卵管炎性不孕症的临床疗效。方法将110例慢性输卵管炎性不孕症患者随机分为3组,治疗组50例于月经第1～5 d予解毒化浊中药双花坤孕胶囊治疗,并于月经干净后4～6 d进行输卵管通液术;对照1组30例于月经干净后4～6 d进行输卵管通液术,并在非经期予解毒化浊中药双花坤孕胶囊;对照2组30例于月经干净后4～6 d进行输卵管通液术,并在非经期予环丙沙星。3组均3个月为1个疗程,治疗1个疗程后观察疗效、输卵管再通情况、用药对月经的影响和沙眼衣原体(CT)、解脲支原体(UU)转阴率,随访2年观察妊娠情况。结果治疗组总有效率96.0%,对照1组总有效率93.3%,对照2组总有效率90.0%。治疗组总有效率高于对照1组、对照2组(P<0.05),治疗组疗效优于对照组。治疗组治疗后输卵管通畅率、总有效率分别为82.0%、92.0%,均高于对照1组(53.4%,76.7%)、对照2组(50.0%,70.0%)。随访2年,治疗组受孕率高于对照1组、对照2组(P<0.05)。治疗组治疗后CT、UU转阴率与对照2组比较差异无统计学意义(P>0.05),治疗组、对照2组治疗后CT、UU转阴率高于对照1组(P<0.05)。3组用药对月经的影响比较差异均无统计学意义(P>0.05)。结论月经期应用解毒化浊中药治疗慢性输卵管炎性不孕症对患者月经情况无影响,用药安全。治疗支原体、衣原体感染,可有效杀灭病原体,明显改善症状,治疗慢性输卵管炎性不孕症疗效肯定。Objective To investigate the effect of Jieduhuazhuo prescription on chronic salpingitis infertility in menstrual period.Methods 110 patients with chronic salpingitis infertility were randomly divided into three groups.50 patients in treatment group were treated by Shuanghua Kunyun capsule in the first 1 ～ 5 d of menstrual period and tubal surgery at 4 ～ 6 d clean after menstruation.30 patients in control group one were treated by tubal surgery at 4 ～ 6 d clean after menstruation and Shuanghua Kunyun capsule in non-menstrual period;30 patients in control group two were treated by tubal surgery at 4 ～ 6 d clean after menstruation and Ciprofloxacin in non-menstrual period;The course was 3 months in three groups.The therapeutic effect,tubal patency,the impact of medication on the menstrual period,negative rate of CT and UU and pregnancy situation of 2-year follow-up were observed after a course of treatment.Results The total effective rate in treatment group was higher than that in the control group one and control group two（P0.05）.The tubal patency rates and pregnancy rates of 2-year follow-up in treatment group were higher than those in the control group one and control group two after treatment（P0.05）.There was no significant difference between treatment group and control group two on negative rate of CT and UU after treatment（P 0.05）.There was statistically significant difference between control group one and treatment group and control group two on negative rate of CT and UU after treatment（P0.05）.There was not statistically significant between three groups on the impact of medication on the menstrual period（P0.05）.Conclusion Application of Jieduhuazhuo prescription on menstrual period did not affect situation of menstruation and has safety on chronic salpingitis infertility.Treatment infection of mycoplasma and chlamydia can effectively kill pathogens and significant improve symptoms,which is effective on the treatment of chronic salpingitis.
