中医辨证分型结合西医治疗重症肺炎47例疗效观察

摘　　要：目的观察中医辨证分型结合西医治疗重症肺炎的临床疗效。方法将95例重症肺炎患者随机分为2组,治疗组47例采用中医辨证分型结合西医常规治疗;对照组48例采用西医常规治疗。2组均10 d为1个疗程,治疗2个疗程后观察中医疗效,并记录2组治疗前后体温、呼吸、心率,白细胞计数、氧合指数、胱抑素-C,X线胸片及胸部CT、痰培养和(或)血培养结果。结果 2组治疗后体温、呼吸、心率、白细胞计数、氧合指数及胱抑素-C均较本组治疗前改善(P<0.05),且治疗组较对照组改善明显(P<0.05)。治疗组治疗后胸部影像总有效率较对照组高(P<0.05),治疗组胸部影像结果优于对照组。治疗组治疗后痰或血培养总有效率较对照组高(P<0.05),治疗组痰或血培养结果优于对照组。治疗组中医疗效总有效率较对照组高(P<0.05),治疗组中医疗效优于对照组。治疗组平均住院天数较对照组短(P<0.05)。结论采用中医辨证分型结合西医常规治疗重症肺炎,能提高临床疗效,缩短住院时间。Objective To observe the clinical effect of syndrome differentiations of traditional Chinese medicine combined with western medicine on the treatment of severe pneumonia.Methods 95 cases of severe pneumonia were randomly divided into two groups.Patients in treatment group（n= 47） were treated by syndrome differentiation of traditional Chinese medicine combined with conventional western medicine.48 patients in the control group were treated with conventional western medicine.The therapeutic course was ten days in two groups.Body temperature,respiration,heart rate,leukocyte count,oxygenation index,cystatin-C,X-ray the chest image,chest CT,sputum or blood culture and efficacy of traditional Chinese medicine were observed after two courses.Results Body temperature,respiration,heart rate,leukocyte count,oxygenation index and cystatin-C after treatment were improved compared with those before treatment in two groups（P0.05）,the improvement of the indexes in treatment group was superior to that in control group（P0.05）.The total efficiency of the chest image in treatment group was higher than that in the control group（P0.05）.The total effective rate of sputum or blood culture in treatment group was higher than that in the control group（P0.05）.The total effective rate of efficacy of traditional Chinese medicine in treatment group was higher than that in the control group（P0.05）.The average length of stay in treatment group was shorter than that in the control group（P0.05）.Conclusion Syndrome differentiations of traditional Chinese medicine combined with western medicine can enhance clinical effect and short length of stay on the treatment of severe pneumonia.
