经阴道三维超声对节育器异常的诊断价值

摘　　要：目的探讨经阴道三维超声对节育器（intrauterine device,IUD）异常的临床诊断价值.方法对临床医生怀疑IUD位置异常的100例患者行经阴道二维超声（2D-TVS）检查后即行经阴道三维超声（3D-TVS）检查,采用表面成像方法,系统观察IUD的位置、形态及与宫腔、肌壁的关系,并和手术结果进行对比分析.结果 100例患者均经手术证实,其中2例为节育器异位至腹腔,27例为断裂节育器或残留节育器残端嵌顿肌层,42例为节育器位置下移,部分伴有嵌顿,29例为节育器旋转、倒置位置异常.三维超声诊断准确率为99%,尤其对IUD在宫腔内旋转诊断准确性明显高于2D-TVS,均能清晰显示IUD特征性三维图像改变.结论经阴道三维超声成像直观清晰,立体感强,定位准确,对诊断IUD异常有非常广阔的临床应用前景.Objective To study the clinical significance of the three-dimensional ultrasonography in the diagnosis of migration of intrauterine devices(IUD).Method 100 patients who were suspected with having migration of IUD were examined with two-dimensional ultrasonography and three-dimensional ultrasonography.The shape and location of IUD and the relationship between endometrium or serometrium and IUD were systemically observed.The ultrasonic observations and the surgical findings were analyzed comprehensively.Results Among 100 cases of ectopic IUD surgically confirmed.The diagnostic accuracy of three-dimensional ultrasonography is 99%,which is significantly higher than the 2D-TVS,3D-TVS which is useful for the IUD rotation in the uterine cavity.Conclusion Three-dimensional ultrasonography,can display direct and clear and three-dimensional images and locate the IUD precisely,which is very helpful for the proper diagnosis of migration of IUD.
