舒芬太尼与芬太尼复合丙泊酚用于无痛人工流产麻醉效果的比较观察

摘　　要：目的比较舒芬太尼与芬太尼复合丙泊酚用于无痛人工流产麻醉的效果。方法将早孕行门诊无痛人工流产患者200例随机分为S组和F组各100例。S组给予舒芬太尼7.5μg静脉滴注,F组给予芬太尼75μg静脉滴注,2min后2组均给予丙泊酚2.5mg/kg静脉推注。连续监测并记录心率(HR)、血压(BP)、血氧饱和度(SpO2),记录最低SpO2、麻醉时间、苏醒时间、丙泊酚累计用量及恶心呕吐等不良反应。结果 2组患者各时点生命体征变化比较差异无统计学意义(P>0.05)。S组最低SpO2高于F组,差异有统计学意义(P<0.05);麻醉时间、苏醒时间短于F组,差异有统计学意义(P<0.05)。2组丙泊酚累积量差异无统计学意义(P>0.05)。S组需托下颌、肢动的发生率均低于F组,差异均有统计学意义(P<0.01)。结论与芬太尼相比,舒芬太尼用于无痛人工流产具有苏醒快、呼吸抑制轻的优点,更能满足门诊人工流产患者手术的要求。Objective To compare the efficacy of sufentanil with fentanyl and propofol anesthesia for induced abortion. Methods 200 cases pregnant women in early stage to be aborted were randomly divided into S group and F group,each of 100 cases. S group used intravenous infusion of sufentanil 7.5lxg,F group used intravenous infusion of fentanyl 75μg,2min after the two groups were given propofol 2.5mg/kg intravenous injection. Continuous monitoring and recording of heart rate （HR）, blood pressure （BP）, oxygen saturation （SpO2）, record the lowest SpO2, anesthesia time, recovery time, total propofol dosage and side effects such as nausea and vomiting. Results Two groups were compared at each time point changes in vital signs were no significant differences（ P 〉 0.05 ）. The lowest SpO2 of S group was higher than that of F group ,the difference was statistically significant（P 〈 0. 05 ） ;Theanesthesia time, recovery time in S group were shorter than those in F group, the differences were statistically significant（ P 〈 0.05 ）. The cumulative amount of propofol was no significant difference（ P 〉 0. 05 ）. Incidences of to be asked jaw and limb move of S group were less than those of F group,the differences were statistically significant（ P 〈 0. 01 ）. Conclusion Compared with fentanyl, sulfentanil can better meet the need of painless abortion, and less in side effect, revive quickly, less respiration inhibition.
