人工肱骨头置换治疗老年肱骨近端严重粉碎性骨折疗效观察

摘　　要：目的探讨人工肱骨头置换在治疗老年肱骨近端严重粉碎性骨折中的疗效。方法采用人工肱骨头置换治疗老年肱骨近端严重粉碎性骨折17例,其中Neer三部分骨折9例,四部份骨折8例。结果 12例获得随访,随访时间6～24个月,平均12个月,伤口无感染、神经损伤、松动、脱位等并发症。肩关节活动度:前屈上举110°,外展70°,外旋30°,按Neer肩关节功能评分标准:优9例,良5例,可2例,差1例。结论人工肱骨头置换治疗老年肱骨近端严重粉碎性骨折,手术时间短,功能恢复快,短期效果满意。Objective To investigate the therapeutic effect on the artificial humeral head replacement for comminuted fractures of the proximal humerus in the elderly.Methods 17 patients with comminuted fractures of the proximal humerus in the elderly were treated with artificial humeral head replacement,9 cases of three-part fractures and 8 cases of four-part fractures,according to Neer classification of fractures of the proximal humerus.Results Twelve patients were followed up for 6~24 months,at an average of 12 months.The range of shoulder joint motion： anteflexion and superduct 110°,abduction 70°,extorsion 30°.According to the Neer score of shoulder joint function,9 cases were graded as excellent,5 as good,2 as fair,and 1 as poor.Conclusion Treatment of comminuted fractures of the proximal humerus in the elderly with artificial humeral head replacement has operative time,quick recovery of function and satisfactory short-term results.
