慢性心衰患者C-反应蛋白、纤维蛋白原、D-二聚体的相关性研究

摘　　要：目的研究慢性心力衰竭患者高敏C-反应蛋白(high-sensitivity C-reactive protein,hs-CRP)、纤维蛋白原(Fibrinogen,FIB)、D-二聚体(D-dimer)的变化、相关性及临床意义。方法选择心衰患者(心功能NYHA分级Ⅱ～Ⅳ级)64例、心功能正常患者21例,检测B型利钠钛(B-natriuretic peptide,BNP)、hs-CRP、FIB、D-二聚体水平,并用彩超测定左室射血分数(left ventricular ejection fraction,LVEF)评定患者心功能。结果①随心衰加重,hs-CRP、FIB、D-二聚体水平明显升高,LVEF水平明显降低;②较心功能正常组,各组心衰患者hs-CRP、FIB、D-二聚体水平显著升高,LVEF水平显著降低(P<0.05～0.01);③hs-CRP与FIB、D-二聚体正相关(r=0.708,0.696 P=0.01);④LVEF与hs-CRP、FIB、D-二聚体负相关(r=-0.669,-0.492,-0.794,P=0.01)。结论慢性心衰患者体内存在炎症反应与高凝状态,两者有相关性,在临床工作中应重视其存在性,并对其干预性治疗予以进一步研究。Objective To observe the changes and correlation between high-sensitivity c-reactive protein（hs-CRP）,Fibrinogen（FIB）,D-dimer in heart failure patients,provide the therapeutic basis for anti-inflammatory and anticoagulant therapy of CHF.Methods Selected 64 heart failure patients of New York Heart Association（NYHA） classification Ⅱ-Ⅲ and 21 patients with normal heart function.All patients were tested B type natriuretic peptide（BNP）,hs-CRP,FIB,D-dimer and echocardiography was applied to measure left ventricular ejection fraction（LVEF）.Results ①Along with the increasing of heart failure,the levels of hs-CRP,FIB,D-dimer were significantly higher,the levels of LVEF were significantly lower;②Compared with normal group,the levels of hs-CRP,FIB,D-dimer in heart failure groups were significantly higher,and the level of LVEF was significantly lower（P0.05~0.01）;③FIB and D-dimer are related with hs-CRP positively（r=0.708,0.696 P=0.01）;④Hs-CRP,FIB and D-dimer are related with LVEF negatively（r=-0.669,-0.492,-0.794 P=0.01）.Conclusions Inflammation and hypercoagulation states are present in the body of heart failure patients.The more serious heart failure,the more serious the inflammation and hypercoagulation states.There is a correlation between inflammation state and hypercoagulation state.So we should take in a certain anti-inflammatory and anticoagulation treatments for heart failure patients.
