摘  要:目的建立简便易行的体外模拟测试方法 ,验证国产镍钛合金自膨式主动脉人工瓣膜支架系统的定位释放性能。方法以直径20 mm带金属标记环的高透明硅胶管模拟的主动脉根部结构,在X线透视下将国产主动脉瓣膜支架插入硅胶管内,模拟支架在体内的释放过程。释放后以硅胶管刻度线为基准,测量释放后瓣膜支架瓣环与基准线的偏差。再按时间顺序将30次释放分为A、B、C组,以A组为对照进行比较。结果成功定位释放30次(100%),准确释放23次(76.67%)。4次定位释放的支架瓣环与基线重合,5次未及基线距离为(-2.00±1.00)mm(-1.00~-3.00mm),21次超过基线距离为(2.84±2.56)mm(1.00~11.00 mm),30次释放总体偏离基线距离为(2.03±3.05)mm(-3.00~11.00 mm)。B组与A组间与基线的偏差距离的差异无统计学意义(P>0.05),但C组与A组间的差异有统计学意义(P<0.05)。结论国产主动脉自膨式人工瓣膜支架系统可控性强、操作简便,以带刻度的硅胶管为模型有助于提高瓣膜支架的定位释放精度。Objective To evaluate the positioning and release properties of domestic nitinol aorta valved stent by a convenient in vitro simulation test. Methods A highly transparent silicone tube (diameter 20 mm) with a metal ring was taken as the simulation of aortic root in the study. A sheath loaded with valved stent was inserted into the tube under digital subtraction angiography (DSA). The vavled stent was released when the prosthetic annulus overlapped the metal ring attached the tube. Then the interval from the prosthetic annulus to the metal ring was measured. The data of interval were divided into 3 groups (Group A, Group B and Group C). Group A (control group) was compared with Group B and Group C, respectively. Results The valved stents were released successfully in the tube 30 times under DSA (success ratio 100%), of which 23 times are accurate (76.67%). Prosthetic annulus overlapped the metal ring 4 times, below the metal ring 5 times (average value [-2.00±1.00]mm, - 1.00-- - 3.00 mm) and surpassed the metal ring 21 times (average value [-2.84±2.56-] mm, 1.00-11.00 mm). There was no significant difference between Group A and Group B (P〉0.05), but the difference between Group A and Group C was statistically significant (P 〈 0.05). Conclusion The present domestic nitinol aorta valved stent is easy and convenient to use. It helps to improve releasing accuracy by imitating the procedure of aortic valved stent in vitro in silicone scaled tube.
