41例复杂性膀胱阴道瘘患者的诊治分析

摘　　要：目的探讨复杂性膀胱阴道瘘的病因、治疗方式及注意事项。方法回顾性分析2000年至2009年我科收治的复杂性膀胱阴道瘘患者的发生原因、术前一般情况、手术方式及治疗效果。结果 41例患者中,妇科、产科因素是导致复杂性膀胱阴道瘘发生的主要原因。手术治疗41例,Ⅰ期修补成功30例,Ⅱ期修补成功8例。术后随访12个月至10年无复发。结论术前充分准备,术中精细操作和术后强化管理是提高复杂性膀胱阴道瘘手术成功率的关键。Objective To investigate the clinical features,diagnosis and treatment for complex vesicovaginal fistulas.Methods The causes,pre-operation condition,operation style and clinical efficacy of 41 patients with complex vesicovaginal fistula from January 2000 to December 2009 were analyzed retrospectively.Results Factors relating to obstetrics and gynecology were the main causes of complex vesicovaginal fistulas.All 41 patients were obtained the operation.Thirty patients were successful treated at stage Ⅰoperation while 8 patients at stageⅡ operation.There was no recurrence during the follow up period of 12 months to 10 years.Conclusion Making sufficient preparations pre-operatively,operating carefully during the operation and strengthening the management post-operatively are the keys to improve the successful rate of the operation for complex vesicovaginal fistulas.
