持续负压引流技术在骶髂关节结核合并腰骶部巨大脓肿外科治疗中的临床应用价值

摘　　要：目的评价持续负压引流技术在骶髂关节结核合并腰骶部巨大脓肿手术治疗中的临床应用价值。方法对2005年10月～2010年10月临床收治的17例骶髂关节结核合并腰骶部巨大脓肿患者行手术病灶清除,术后应用持续封闭负压引流7～10天,观察在院治疗效果和愈后复发情况。结果 17例病人全部获得随访,随访时间6～38个月,病人伤口均一期愈合,术后无结核病灶复发。结论骶髂关节结核合并腰骶部巨大脓肿在手术清除脓肿病灶后,持续封闭负压引流技术的应用能够使创腔内炎性积液排空干净,并能确保切口愈合、防止术后复发,在骨与关节结核的临床治疗中拓展了技术空间,具有重大的临床应用价值。Objective To evaluate the clinical effect of the vacuum sealing drainage technology in the debridement surgery of sacro- iliac joint tuberculosis with large lumbosacral abscesses. Methods From october 2005 to october 2010, 17 sacroiliac joint tuberculosis patients with large lumbosacral abscess underwent debridement using vacuum sealing drainage and after that they were continuing attracted for 7 to 10 days. Patients situation were observed in the hospital after treatment. Results 17 patients were followed up for 6 to 38 months. All the patients healed. There was no postoperative recurrence of tuberculosis. Conclusion In surgical removal of lesions on sacroiliac joint tuberculosis with large lumbosacral abscess, the continued application of vacuum sealing drainage （VSD） can create a clean cavity, and ensure wound healing and prevent recurrence. It expands technological space and has a great value in the clinical treatment of bone and joint tuberculosis.
