女性人乳头瘤病毒感染及基因型别分析

摘　　要：目的了解绍兴地区女性人乳头瘤病毒(HPV)各基因型感染检测的分布情况,以探讨HPV感染的临床意义。方法以核酸分子快速杂交仪为平台,利用导流杂交原理,样本经过PCR扩增后可同时检测21种HPV亚型的基因分型,包括15种高危亚型和6种低危亚型。结果 503例女性标本中,共检出阳性标本217例(43.14%),毒株亚型19种332株(含多重感染的重复计算);其中,低危亚型123株,高危亚型209株。低危亚型HPV感染主要以HPV6(18.98%)、HPV11(15.96%)为主,高危亚型主要以HPV16(12.95%)、HPV58(11.14%)、HPV52(10.84%)、HPV18(4.82%)等为主。检出单一感染HPV亚型145例(28.83%),多重感染72例(14.31%),包括二重感染48例(9.54%),三重感染16例(3.18%),四重及以上感染8例(1.59%),感染亚型最多的达9种。检测人群以21～40岁为主,最多的是21～25岁,感染高峰则出现在<25岁和>50岁以上两个年龄段,感染率远高于43.14%的平均水平。不同年龄群的感染均以单一感染为主,但感染有年轻化和低龄化的趋势。结论绍兴地区女性宫颈HPV感染率较高,开展各型HPV检测对于HPV感染的诊断、治疗及子宫颈癌的早期发现、预防和治疗具有重要意义。Objective To report the genotype prevalence and distribution of human papillomavirus（HPV） in women who were suspicious as HPV infection by clinical diagnosis in Shaoxing,and to investigate its clinical significance.Methods A low density gene chip containing 21 genotypes including 15 high risk types and 6 low risk types was detected by flow through rapid hybridization after HPV DNA amplified by PCR technology.Results Totally 217 samples（43.14%） were HPV positive among 503 clinical specimens,and 19 HPV genotypes were detected except HPV42 and 43.HPV6（18.98%） and 11（15.96%） were the most frequent low risk subtypes,while HPV16（12.95%）,HPV58（11.14%）,HPV52（10.84%）,HPV18（4.82%）were the high risk subtypes.Among 217 positive samples,145 cases（28.83%） were HPV single infection and 72 cases（14.31%） were multiple infection,including 48 cases（9.54%） double infection,16 cases（3.18%） triple infection and 8 cases（1.59%） more than triple infection.Women with clinical detection of HPV infection were mainly from 21 to 40 years old,and most are from 21 to 25 years old.The peak infection was found in two groups： younger than 25 years old and older than 50 years old.Their positive rates were far higher than average.Most women were single infection even in different age group,but HPV infection trend was towards younger.Conclusion The HPV infection rate is high correspondingly in Shaoxing,and the determination of HPV genotypes plays an important role in early discovery,prevention and treatment of female genital tract diseases,such as condyloma acuminatum,cervical cancer and so on.
