摘　　要：目的:观察重组改构肿瘤坏死因子(recombinant mutant human tumor necrosis factor,rmhTNF)及白介素-2(interleukin-2I,L-2)胸腔内给药治疗非小细胞肺癌(NSCLC)并胸腔积液患者的近期疗效及不良反应。方法:回顾性分析经胸水或胸膜细胞学确诊为NSCLC并胸腔积液的患者共60例r,mhTNF联合IL-2组(A组)36例I,L-2(B组)24例,患者均应用吉西他滨+顺铂方案化疗,观察2组的疗效和不良反应结果:A组有效率(80.6%)高于B组有效率(50.0%),2组比较差异有统计学意义(P<0.05),不良反应有发热和胸痛。结论:rmhTNF联合IL-2胸腔内注射治疗NSCLC并胸腔积液优于单用IL-2,值得临床推广应用。Objective： The aim of our study was to explore the short-term effects and complication of chemotherapy combining with interleukin-2（IL-2）and recombinant mutant human tumor necrosis factor（rmhTNF）in the treatment of malignant hydrothorax. Methods： 60 cases patients with malignant hydrothorax were randomly divided into two groups.36 examples were given thoracic cavity injection IL-2 and rmhTNF as group A,24 cases in group B using rmhTNF alone.All of the patients were treated with Gemcitabine combining with Nedaplatin for chemotherapy.The effects and complications were observed. Results： The total response rate in group A was higher than that in group B（80.6% vs.50.0%）,which had statistically significant difference（P0.05）.The complications included chest pain and fever. Conclusion： The therapy of IL-2 combining with rmhTNF intra-thoracic injection for malignant hydrothorax could achieve better effects,which is worth to be used in clinic.
