摘　　要：目的:应用定量组织多普勒技术(QTVI)评价2型糖尿病患者的左室收缩舒张功能,为糖尿病性心肌病的早期预防和诊断提供可靠依据。方法:研究资料分为两组,2型糖尿病患者(DM组)和健康体检人员(NC组)各30例,应用QTVI测量二尖瓣环6个位点的收缩期峰值速度(Sa)、舒张早期峰值速度(Ea)、舒张晚期峰值速度(Aa)、舒张早期与舒张晚期峰值速度比值(Ea/Aa),计算6个位点平均值代表瓣环的整体运动。结果:二尖瓣环运动速度Sa在DM组与NC组之间比较差异无显著性,Ea、Ea/Aa在DM组显著低于NC组。Aa在DM组与NC组之间比较差异均无显著性。结论:QTVI可以敏感、准确地评价2型糖尿病患者的左室功能,糖尿病患者左室舒张功能受损较早,随后才出现左室收缩功能减低。Objective To assess left ventricular systolic and diastolic function in patients with type 2 diabetes mellitus by qantitative tissue velocity imaging, and to provide reliable evidence for early prevention and diagnosis in diabetic cardiomyopathy. Methods The population of the study consisted of 2 groups ： 30 patients with type 2 diabetes mellitus （DM） and 30 control subjects （Control）. Six positions of mitral annular were measured by QTVI including peak systolic velocity （Sa）, peak early diastolic velocity （Ea）, peak end diastolic velocity （Aa）, and Ea/Aa ratio, from which the average value was taken. Results Sa were not significantly different in the DM compared with the control. Ea, Ea/Aa significantly decreased in the DM compared with the control. There were no significant differences in Aa between the 2 groups. Conclusion QTVI is a sensitive and accurate tool to evaluate LV function in diabetic patients. Diastolic dysfunction appears early in the natural history of type 2 diabetes mellitus and impaired systolic function may subsequently become apparent.
