老年高血压病合并糖耐量异常患者外周血管健康的评价  

Value of PWV, ABI and IMT Test on Assessment of Vascular Health Status in Elderly Hypertensive Patients with Impaired Glucose Tolerance

摘　　要：目的应用PWV、ABI、1MT指标对老年高血压合并糖耐量异常患者外周血管健康进行评价。方法根据血压和口服葡萄糖耐量试验结果，将受试者分为A组（老年高血压病合并糖耐量异常）、B组（老年高血压病）和C组（正常对照组），观察3组baPWV、ABI、IMT及颈动脉斑块检出率。结果A组病人baP—WV、IMT显著高于B组（baPWV1895．31±268．46VS1723．65±247．38，P〈0．01；IMT0．96±0．18V80．75±0．14，P〈0．01）。两组ABI值也具有显著性差别（1．01±0．19VS1．15±0．14，P〈0．01），其中A组斑块检出率为68．1％，B组斑块检出率为47．6％，两者具有显著性差异（X^2=4．58，P〈0．05），两者的ABI值也具有统计学差异（0．93±0．12VS．1．08±0．10，P〈0．01）。结论老年高血压易发生外周动脉损害，合并糖耐量异常时尤为明显，无创性baPWV、ABI和IMT指标可较好地评价血管健康状态，可用于健康管理。Objective To evaluate the vascular health status in elderly hypertensive patients with impaired glucose tolerance. Methods We assessed ABI,ba - PWV and IMT in 47 elderly hypertensive patients with impaired glu- cose tolerance and 63 age, and gender - matched hypertensive patients. ABI and baPWV were measured by a non - invasive automatic waveform analyzer. Results baPWV was significantly higher in elderly hypertensive group with impaired glucose tolerance than that in elderly hypertensive group（ 1895.31 ± 268.46 vs. 1723.65±247.38 cm/s ） and the mean ABI value in elderly hypertension with impaired glucose tolerance group（ 1.01 ±0.19）was lower than that in elderly hypertensive group（ P 〈 0.01, respectively）. IMT level was obviously elevated in elderly hypertensive patients with impaired glucose tolerance than that in elderly hypertensive patients （0.96 ± 0.18 vs 0.75 ±0.14,P 〈 0.01 ）. Further study showed that the rate of carotid plaques was obviously higher in elderly hypertensive patients with impaired glucose tolerance than that in hypertensive patients （ 68.1% vs. 47.6%, P 〈 0.05 ）, and their ABI level were significantly different （0.93 ± 0.12 vs 1.08 ± 0.10, P 〈 0.01 ）. Conclusion These findings suggest that peripheral vascular damage is more obviously observed in elderly hypertensive patients with impaired glucose tolerance than that in elderly hypertensive patients. The untraumatic tests of ABI,baPWV and IMT are very helpful on assessment of vascular health status, and they fit for the request of health management.
