胸大肌肌皮瓣修复头颈部肿瘤术后组织缺损的护理

摘　　要：目的:总结胸大肌肌皮瓣修复头颈肿瘤术后组织缺损的护理方法。方法:对2000年3月—2010年8月在头颈部大面积手术缺损后进行的胸大肌肌皮瓣修复术患者77例进行严密的病情观察及精心护理。结果:77例胸大肌肌皮瓣修复患者中72例成活,其中边缘坏死2例,经修剪清创换药,伤口愈合。6例发生伤口感染,经换药、引流、修剪坏死组织后愈合。5例胸大肌肌皮瓣发生全部坏死,剪除坏死组织,换药待新鲜肉芽形成后用邻近组织瓣转移修复。结论:术后精心护理有利于皮瓣成活,提高手术成功率,减轻患者心理负担,提高生存质量。Objective：To study the postoperative nursing experiences of pectoralis major myocutaneous flap in the reconstruction of parenchyma defect for head and neck cancer.Methods：A total of seventy-seven patients,who were reconstructed with pectoralis major myocutaneous flap from March 2000 to August 2010 in our hospital,were strictly observed andcarefully nursed.Results：Among them,72 cases were cured.2 cases was borderline necrosis,the wound was cicatrized after clipping,debriding and changing medical.6 cases has wound infection,the necrotic tissue was cicatrized after changing medical,drainaging and clipping.The figuration and function of tissue defect region were obviously improved.The total flap necrosis appeared in five cases.The figuration and function of tissue defect region were obviously improved.The total flap necrosis appeared in five cases.Conclusion：Careful nursing increased the flaps survival rate and surgery success rate after operation.In the meanwhile,it can alleviate patients′ psychological burdens,and improved the patients quality of life.
