非那雄胺对TURP术中出血影响及其机制的Meta分析

摘　　要：目的探讨术前口服非那雄胺对经尿道前列腺电切术(TURP)术中出血的影响和相关机制。方法收集国内外已发表的有关术前口服非那雄胺对TURP术中出血影响及相关机制的随机对照研究(RCT),利用RevMan软件进行固定效应模型和随机效应模型的Meta分析。结果共纳入13篇RCT研究,包括1 050例患者,其基线情况具有可比性。术前口服非那雄胺组和对照组相比,术中出血量、切除每克前列腺组织出血量、前列腺组织中血管内皮生长因子(VEGF)表达水平和前列腺组织微血管密度(MVD)均显著降低(P均<0.01)。结论术前口服非那雄胺能减少TURP术中出血,可能与降低前列腺组织中VEGF表达,减少前列腺组织MVD有关。Objective To evaluate finasteride given pre-operation on decreasing blood loss during transurethral resection of prostate（TURP） and its possible mechanisms.Methods The literature search strategy was followed based on the Collaborative Review Group search strategy.Published data of randomized controlled trials referred to effect of preoperative finasteride on decreasing bleeding during TURP and its possible mechanisms were analyzed.Results A total of 13 trials involving 1 050 patients were indentified in the Meta-analysis.And based on the analysis,total blood loss and bleeding per gram resected prostate tissue of finasteride group were significantly less than control group（P0.01）.Vascular endothelial growth factor（VEGF） and microvessel density（MVD） were less in finasteride group as well（P0.01）.Conclusion Preoperative finasteride could decrease bleeding during TURP,and its possible mechanisms might be that finasteride decreased the expression of VEGF and MVD.
