摘　　要：目的研究外科术后危重患者外周血中单核细胞表面HLA-DR抗原表达水平与预后的关系。方法将25例ICU中外科术后危重患者根据预后分为好转组和死亡组,并取25例健康成人血标本作为正常对照组,采用免疫荧光方法连续监测其外周血单核细胞人类白细胞抗原DR(HLA-DR)的表达。结果与对照组相比,外科术后危重患者外周血单核细胞HLA-DR的表达水平较低(P<0.05)。入ICU后第4天和7天的好转组和死亡组HLA-DR表达水平具有差异性(P<0.05;P<0.01)。结论术后危重患者外周血中单核细胞表面HLA-DR抗原表达水平与预后具有相关性。Objective To explore the HLA-DR expression on monocyte in peripheral blood of ICU critically ill patients after surgery and observe its relationship with prognosis of patients. Methods HLA-DR expression on monocytes in peripheral blood was measured in 25 critically ill patients after surgery by using flow cytometry after entered the ICU, and were compared with those in the 25 healthy volunteers. Results The HLA-DR expression on monocyte in peripheral blood in critically ill patients after surgery were lower than that of healthy volunteers（P〈 0.05）. The monocyte HLA-DR expression of survivors on day 4 and 7 after entered ICU was higher than non-survivors. Conclusions The expression of monocyte HLA-DR in critically ill patients after surgery was correlated with prognosis.
