摘 要:目的:探讨妊娠期糖尿病(GDM)患者腹部皮下和网膜脂肪组织中Omentin基因表达的差异及其与胰岛素抵抗的相关性。方法:将43例经75g口服葡萄糖耐量试验按WHO标准诊断出的GDM孕妇与43例糖耐量正常(NGT)孕妇进行年龄、产次、分娩前体重指数(BMI)、分娩孕周配对后,测量两组产前空腹血糖和胰岛素水平,并于剖宫产术中留取研究对象腹部皮下和网膜脂肪组织,采用实时定量-聚合酶链反应(RT-PCR)检测不同部位脂肪组织的Omentin mRNA表达水平,分析Omentin mRNA表达水平与产前胰岛素抵抗的相关性。结果:(1)GDM组皮下及网膜脂肪组织的Omentin mRNA表达水平均显著低于NGT组对应部位的表达水平。(2)单因素分析中,HOMA-IR与孕前和分娩前BMI、以及皮下和网膜脂肪组织的Omentin mRNA表达水平均有显著相关性;但进一步多元逐步回归分析显示,只有孕前BMI和网膜脂肪组织的Omentin mRNA表达量是HOMA-IR的独立预测因子。结论:GDM患者网膜脂肪组织Omentin mRNA表达水平降低可能参与了妊娠期晚期IR的发生。Objective:To investigate the changes of omentin mRNA expression in different parts of adipose tissue in gestational diabetes mellitus(GDM)and normal glucose tolerance(NGT)and the relationship between the omentin mRNA expression with insulin resistance.Methods:In a prospective study,43 women identified with GDM according to WHO criteria by a 2-h 75g oral glucose tolerance test were compared with control group matched for age(±1year),exact parity,body mass index(BMI),and gestational weeks at delivery(±1week).Abdominal subcutaneous(SA)and visceral adipose(VA)tissues were obtained during cesarean section.Real-time quantitative PCR was used to measure the omentin mRNA level in adipose tissues.The omentin mRNA expression in different parts of adipose tissue in two groups were evaluated and the relationship between omentin mRNA expression and insulin resistance were analysed.Results:(1)In GDM omentin mRNA expression in both SA(0.057±0.014 vs 0.070±0.015,P0.01)and VA(0.560±0.125 vs 0.713±0.150,P0.01)were significantly lower than those in NGT.(2)By univariate analyses,HOMA-IR was found to be related with BMI at pre-pregnancy(r=0.406,P=0.001)and delivery(r=0.332,P=0.002),as well as with omentin mRNA expression in SA(r=-0.277,P=0.010)and VA(r=-0.228,P=0.035)statistically.However,when further multiple stepwise regression analysis was used,only BMI at pre-pregnancy and omentin mRNA in VA were proved as independent predictors of HOMA-IR.Conclusions:The decrease of Omentin mRNA expression in visceral omental adipose tissue of GDM patients may be involved in the deterioration of IR during the late pregnancy.
